As Vice-President, RN Enterprise Care Continuum Services you will*:

  • Support continued development and implementation of Care Continuum tools and programs designed to impact cost, quality, and experience – Discharge Decision Algorithm, Informed Choice Process, Accountable Care Management, etc.
  • Integrate and advance Care Continuum Services into Navvis’ Population Health services - Physician Optimization Program, Care Coordination, Post-Acute Care Networks, Patient Assessments, etc.
  • Work with the Navvis Analytics team to request client-specific data intended to understand client business trends related to Care Continuum Services – Emergency Department Utilization, Inpatient Discharge Decisions, Home Health, Home Care and Advanced Home Services including Hospital and SNF at Home.
  • Collaborate internally and with client executives, physicians, and post-acute entities including physician groups, health systems, home service providers and social service agencies to create best in class processes and services to meet the immediate and strategic needs of the client. This includes conducting interviews with key stakeholders (executive leadership, client management, staff and physicians, post-acute organizations etc.) to identify themes and help establish current state clarity
  • Provide post-acute care clinical and operational expertise and practical examples,
  • Analyze industry data and prepare reports of trends in performance on a market, client, regional and national level
  • Monitor updates related to Care Continuum Services from NCQA, CMS, Joint Commission, etc. and provide overview to team members
  • Research best practices related to Care Continuum Services for inclusion in Navvis tools such as interview guides, surveys, scorecards, gap analyses, care pathways, etc.
  • Lead teams and oversee the work of other team members in the fulfillment of various aspects of client project engagements
  • Plan, coordinate, develop and deliver high quality, specialized documents, written reports, and presentations for client organizations

. Requirements: We are excited about you if you have these things:

  • Ten or more years in leadership and/or advisory experience in Care Continuum Services. Highly prefer recent experience in post-acute care services. For example, skilled nursing, LTAC, IRF, and/or home health industry
  • Clinical License – nursing, therapy, pharmacy, etc.
  • An understanding of healthcare economics and regulatory requirements governing skilled nursing/home health, BPCI A, MSSP/ACOs, CMS Waiver Programs, Value Based Purchasing Programs, etc.
  • Skill in organizing and prioritizing work, effectively managing multiple projects, delegating, and achieving goals and objectives
  • Skill in exercising a high degree of initiative, professionalism, judgment, and discretion
  • Ability to work effectively Independently and as part of a team
  • Strong critical thinking, problem identification and problem resolution skills
  • Superior verbal and written communication skills
  • A focus on detail and accuracy of work
  • Strong business acumen
  • Willingness to travel up to 50-75%
  • St. Louis based preferred but not required
