Job Description

Envision provides a variety of services to support individuals with disabilities and has been a trusted resource for families for over 50 years. Our mission is to support a full and vibrant future for people with disabilities. In order to better fulfill this mission, Envision made the conscious decision to incorporate the Sanctuary Model across all of the various programs in our agency. The goal of adopting the Sanctuary model was to create a compassionate, trauma informed environment that promotes healing and recovery. We serve more than 450 children and adults each year. Our programs serve children and adults and address a range of conditions and disabilities including: physical or cognitive disabilities, syndromes associated with intellectual disabilities, autism spectrum, learning disabilities, and co-occurring mental health issues. Envision is a contract agency of Hamilton County Developmental Disabilities Services, Butler County Board of Developmental Disabilities, and Clermont County Board of Developmental Disabilities.

Position Overview:

As part of a supportive team focused on excellence and ongoing development and learning, the Care Coordinator will join Envision, an organization with a rich history of serving the needs of individuals with developmental disabilities. The Care Coordinator will be required to develop meaningful and person centered relationships with the individuals served and their family members and create customized supports for each individual that embraces positive behavior supports and engagement in a full and meaningful life. The Care Coordinator’s primary duties are management and handling general business operations of Envision, including personnel management, safety and health related duties, and regulatory compliance.

These primary duties referenced above include the exercise of discretion and independent judgment with respect to matters of significance. Some of these matters of significance in which the Care Coordinator exercises discretion and independent judgement include providing oversight and coordination of all services delivered by their team for the individual’s care and day to day needs; developing a customized, engaging milieu for individuals including group and individualized activities; supervising and training direct support professionals in the care of individuals receiving services; providing support to adults receiving services in the development of: coping skills, social skills, communication skills, activities of daily living skills, and expansion of leisure interests and skills; coordinating care and support schedule of the staff, ensuring coverage of all scheduled times of day with respect to the approved service plan; providing high level collaboration and communication with all service providers and family members; and money management.

Care Coordinators must be able to supervise others, multi-task, and lead by example, with dedication, positive attitude and support to develop a good working rapport with the individual, family members and other support team members. The Care Coordinator reviews all service documentation for compliance with all applicable rules, regulations and standards.

ESSENTIAL FUNCTIONS AND JOB DUTIES

FACILITIATION AND DELIVERY OF SERVICES

  • Coordinates care to individuals served in assigned homes as indicated in individuals’ service plans (ISP). Services include skill development, homemaker personal care, outside activities, work schedule, and may include additional services such as nutrition/diet planning, and other services as described in the ISP.
  • Ensures effective flow of communication with individuals, family members, staff, co-workers, other professionals and community members through effective verbal and written communication skills, and successful team collaborations.
  • Obtains monthly and intermittent medication deliveries from CLS Nursing staff as appropriate. Reviews medications received for accuracy and report any discrepancies immediately. Works with nursing staff and pharmacies as needed. Delivers unused medications to nursing staff monthly for proper reporting and destruction.
  • Ensures that individuals’ specific dietary needs are met by developing appropriate meal plans and menus with instructions and oversights with staff in the preparation of meals. Develops, implements and monitors meal planning/preparation. Works with CLS Household Supports Specialists (HSS) and other CLS staff regarding dietary needs and menu planning.
  • Make purchases for non-routine groceries, personal, clothing and others items as needed.
  • Coordinates services for individuals by working with the individual and his team, including physicians, service facilitators, nursing staff, and guardian/family members in developing individual service plans and facilitating ongoing relationships and coordination of care.
  • Attends all Service Plan meetings to ensure they are held at least 45-60 days prior to span expiration. Prepare for new plans by meeting with Support Associates and family members to obtain information to be incorporated into new plans. Notifies CLS Administrative staff of scheduled My Plan meetings at least one week prior to meeting. Work with CLS Administrative Team in scheduling meetings to assure timely receipt of new plans before existing spans expire.
  • Develops and Manages household budgets Manages individuals’ personal funds and maintain accurate records and receipts in. Submits receipts and other financial information to Finance staff in a timely manner. Cash reconciliation will occur on the first business day each month.
  • Manages records and coordinate preparation for benefits review meetings. Notifies CLS Administrative staff when benefits meetings have been scheduled, at least one week prior to meetings. Attends benefits meetings as needed.
  • Regularly monitors individuals’ health status and arrange for medical, dental, vision and other appointments as needed. Attend physician appointments. Notifies nursing staff of scheduled appointments and request LPN to accompany when available. If unable to attend the meeting, the staff person assigned must be approved by the nursing staff. Floater positions may also be used, when available to assist with doctors appointments. Report all doctors, dentists and care provider appointments when scheduled and all hospital visits (ER and admitted stays) to the Nursing staff immediately.
  • Plans and coordinates activities and outings for individuals. Consults with the SL Director and/or Trainer to develop activities and skill development plans and staff role in supporting individuals in working towards these goals. Assure adequate staffing for outings. Arranges transportation to day/work programs and outings as needed.
  • Conducts monthly emergency drills as required per established procedures. Submits drill reports to CLS Administrative staff as required.

ADVOCACY

  • Ensures observance and facilitation of the individual’s rights.
  • Identifies advocacy issues and brings to team/supervisor.
  • Facilitates, assists, and/or represents the individual served when there are barriers to his/her service needs.
  • Interacts and educates community members and organizations (landlord, employer, etc.) when relevant to the individual’s needs or services.

CRISIS INTERVENTION

  • Regularly monitor individuals’ health status and reports changes to usual health status to the nursing department immediately for additional assessment and follow-up.
  • Ensure implementation by staff of positive behavior supports during times of crisis. Proactively develop support plans when behaviors that have the potential to become unsafe occur.
  • Reports medical emergencies and other unusual incidents according to established procedures for incident reporting. Makes necessary notifications to family/guardian and Envision Nursing staff. Assures staff remains update on incident reporting procedures. Completes Incident Reports and Individual Status Report with 1 business day (within 4 hours for Major Unusual Incident).
  • Maintains a working smart phone (monthly reimbursement by agency, per established rate) to assure availability for on-call coverage for assigned homes to assure adequate staffing, respond to staff questions, supervise and manage unusual incidents and respond to calls from staff and/or other staff, as well as family members, physicians and others involved parties. On-call coverage will be arranged on a Rotation Basis with other Care Coordinators.

SCHEDULING:

  • Care Coordinators are on-call on a 24/7 basis, with on-call coverage arranged with a rotation plan with other Care Coordinators.
  • Approximately 70% of the work week Care Coordinators will be present in the homes for during varied weekday and weekend service delivery. The Care Coordinator should be present in the home during service delivery hours and routinely available during scheduled family visits.
  • Care Coordinators meet regularly on campus one day per week for staff meetings, scheduling updates, training and coordination.

REQUIRED TRAINING FOR THE CARE COORDINATOR ROLE:

  • Complete Training Course on Positive Behavior Supports demonstrating competence in all primary concepts of Positive Behavior Support and ability to apply those concepts to real-life scenarios but theoretically and in the moment.
  • Complete Training Course of Functional Behavior Assessment demonstrating the ability to complete a basic behavioral assessment independently, including identifying data to track, developing data tracking logs, and implementing comprehensive data tracking processes.
  • Complete Training Course on Relational De-Escalation demonstrating ability to successfully pass all content areas of the training as identified in the post-test.
  • Complete Training Course on Task Analysis and Skill Development demonstrating ability to apply the primary concepts by completing an analysis and developing a skill development plan.
  • Complete Training Course on Skill Development Planning and Intervention Methodology demonstrating ability to write a basic skill development plan that includes preventative, teaching and reaction steps when implementing a skill development plan.
  • Develop and Support Staff in Implementation of a Leisure/Recreational Milieu for each CLS site that reflects the individual needs of each individual living in the home based on their abilities and interests.
Qualifications

MINIMUM JOB REQUIREMENTS

Position Requirements:

  • Required minimum qualifications:
  • Bachelor’s in Psychology, Social Work or Special Education (five years of experience in the field will be considered in substitute of the education requirements)
  • An insured, reliable vehicle for possible on-the-job use

Physical Requirements:

  • Ability to regularly perform physical activities such as lifting, carrying, pushing and pulling 50 lbs.
  • Bending, transferring, stooping and walking when needed.

Required Experience:

  • At least 2 years in the field working with individuals with developmental disabilities, mental health diagnosis or co-occurring developmental disabilities and mental health diagnoses
  • At least 1 year supervisory experience.

Preferred Experience:

  • Master’s in Psychology, Social Work, Special Education or Clinical Counseling
  • Experience as a mental health technician
  • Experience with applied behavioral analysis
  • Experience supporting individuals in hospital, mental health treatment or school settings
  • Experience supervising others
