STATUS: Flex / PRN
JOB RESPONSIBILITIES:
Provides direct social work services to long term care patients and families. Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning. Assumes responsibility for required documentation in compliance with State and federal regulations.
QUALIFICATIONS:
LGSW/LCSW/LCSW-C in the State of Maryland.Ability to perform duties with professional competence and independence.Experience in hospice or medical setting.Demonstrated computer skills.
