The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Roles & Responsibilities:Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services takes a leadership role in MSHP Care Management, which is providing in person and telephonic short-term and long-term care management services for a large diverse group of individuals who have a variety of insurances that are part of value based contracts. The goal of MSHP Care Management is to provide standardized care management services to meet patients’ medical and psychosocial needs, increase quality and reduce unnecessary health care costs.

The Social Worker in the Institute for Advanced Medicine (Comprehensive Downtown Program) provides continuous, coordinated care to patients and their families throughout the course of their illness. They will provide a full range of social work services to patients living with HIV, as well as to their partners and family members. Services provided include: crisis intervention; assistance with entitlements; short-term and ongoing individual, family and group counseling focusing on HIV treatment adherence and psycho-education; and facilitating linkages to community case management. The Social Worker is involved in outreach activities throughout the Mount Sinai health system as well as with community-based organizations such as the HIV and AIDS Services Administration (HASA) and the Gay Men’s Health Crisis. The Social Worker will work closely with a Care Coordinator to ensure collaborative and seamless work. The Social Worker may also be responsible for making home visits and/or escorting patients to agency appointments. The Social Worker will also be responsible for attending regular meetings with MSHP CM. Finally, this position requires strong attention to documentation and flexibility to understand evolving administrative requirements.

This position is based at Mount Sinai’s Comprehensive Downtown Program, located at 275 7th Avenue (12th floor), between 25th and 26th Streets, Manhattan, New York City.
Requirements:
  • Experience working with the LGBTQ community and adults who are HIV+ is preferred.
  • Experience working with medically complex patients.
  • Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
  • Strong attention to administrative details.
  • Excellent engagement, collaborative and clinical skills.
  • Bilingual (Spanish English) preferred.
