Job Description
Performs comprehensive functional mobility assessments within their scope practice to
evaluate a member's capacity for ADL/IADLs to inform the need for PCA services.
Assessments can be performed for initial requests for services in conjunction with a CCA
OT, for reassessment of existing services set to expire, or a reassessment in the case of a
status change
  • Assess member's ability to manage the PCA program
  • Performs detailed medication inventory
  • Identifies and relays DME needs to CP
  • Suggests appropriate LTSS based on assessment
  • Create associated authorization if PCA services are deemed appropriate
  • Collaborate with CCA Care Partner, GSSC, and community?based PCPs/Specialists, as
needed
  • Communicates identified concerns or needs for skilled care with CCA interdisciplinary
site team to ensure comprehensive member needs are consistently met (e.g. behavioral
health, health outreach, chronic disease management, etc.)
  • Documents all activities and results using an Electronic Medical Record, in an effective
manner, while strictly adhering to CCA policies and procedures
  • Engages in collaboration with peers across CCA sites to support implementation,
education and standardization of practices
  • Provide clinical care to members via telehealth technologies (video, chat, etc.) for a
clinically appropriate clinical care and care management services.

Required:
  • Associate's degree in nursing required,
Preferred:
Bachelor's Degree in nursing preferred

Required:
  • Registered Nurse with licensure in good standing inn Massachusetts
  • Meaningful clinical experience in primary care, care management and/or LTSS,
including:
? 5+ years as a Registered Nurse in a high touch clinical environment or home care; OR
? 2+ years caring for patients/members with complex medical, behavioral health, and
social needs
  • Certified in Basic Life Support for Healthcare Providers
  • Must be willing and able to travel to members' homes in addition to working in an office
environment occasionally
  • Valid driver's license with no restrictions. Ability to be active and mobile across
Massachusetts
  • Languages Required -- English, bilingual preferred
  • Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate,
for a variety of clinical care and care management services.
