Description:

The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Team caring for patients and families residing in local nursing homes. Our RN Home Care case managers coordinate with an interdisciplinary team and the nursing home team to develop a comprehensive plan of care to provide added support to patients and families. Training provided in end of life care.


This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).


Work days are between Monday to Friday 8am to 4pm.

Minimum Requirements:

  • Valid NYS RN license
  • Associates degree in nursing
  • 1-2 years experience as a RN (acute care preferred but not required)
  • Valid driver license and reliable vehicle

Preferred Requirements:

  • 1+ year of Hospice or Home care experience
  • Bachelor's degree in nursing
  • Understanding of Hospice principles
  • Maintain the highest level of customer service while completing day to day duties and responsibilities
  • Highest level of interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers
  • Passion to learn, grow and develop in the nursing field
