Make visits to patients in their homes and perform skilled care activities. Plans and directs services to meet the needs of individuals and families within their homes and communities. Computer skills and 1-2 years of recent acute care experience in an institutional setting or home health experience required. Oasis and Medicare experience preferred. Weekdays with some early evenings and some weekends.
