Primary Purpose:
Assesses the needs of patients. Develops a plan of intervention that is integrated with the interdisciplinary treatment team. Increases patient/family satisfaction. Identifies age related issues. Implements plan of care. Provide referral and linkage to health care and community resources to assure continuum of care for patients. Communicates home care needs to home health agency. Provides clear written communication of plan of intervention for patient. Accountable for timely comprehensive documentation of all professional services. Based on department standards, completes brief case sheets and closes cases in a timely and accurate manner.
Qualifications
Knowledge and Skills:
Education: LPN, RN, BSW; Bachelor's or Master's degree preferred.
Required Skills and Knowledge: Homecare experience strongly preferred. Must be flexible since position is an as needed only position. Organization - West Chester Hospital Primary Location - West Chester Hospital - 7700 University Dr - West Chester, OH
