RN Case Manager-Per Diem Visits Home Care

We need RN Case Managers with a passion for home care to provide leadership and education so more well-trained CNA's can reach more seniors who desire the freedom to stay at home. This is a flexible position. Case Managers follow clients through care and have the flexibility to schedule visits independently within certain time frames. Our pay rates at $90/visit are one of the most competitive you will find. And for good reason.
