The RN Community Relations Coordinator participates in hospital/nursing home interdisciplinary staffing meetings, coordinates referrals and identifies skilled care needed. Oversees and coordinates the patient schedule for nursing, therapies, and nursing assistants to assure patient outcomes are met. Schedules admits and coordinates nursing visits based on the patient’s acuity so that staffing patterns are maintained. Participates with multidisciplinary team and referral sources to develop, evaluate, revise, and implement the comprehensive plan of care and coordinates discharge planning. Functions as a resource to staff regarding clinical and patient care issues, procedures, and regulatory requirements.







Essential Job Functions
Title and Description

Participates in hospital/nursing home interdisciplinary staffing meetings, takes referrals, initiates and completes intake, and identifies skilled care needed. Obtains appropriate authorization for a verified payer source.


Participates with multidisciplinary team and referral sources to develop, evaluate, revise, and implement the comprehensive plan of care. Coordinates discharge planning.


Oversees and coordinates the patient schedule for nursing, therapies, and nursing assistants to assure patient outcomes are met. Schedules admits and coordinates nursing visits based on the patient’s acuity so that staffing patterns are maintained.


Facilitates, coordinates, and monitors student’s/resident’s clinical experience. Facilitates staff involvement in nurse/department meetings assuring that staff are provided with necessary information.


Participates in performance improvement, Outcome Based Quality Improvement (OBQI), and review of documentation to assure satisfaction, physical, and financial outcomes of the patient.


Functions as a resource to staff regarding clinical and patient care issues, procedures, and regulatory requirements. Performs skilled nurse and aide supervisory visits and coordinates education efforts.


Serves on internal and external committees or task forces relevant to the improvement of Home Health and Hospice programming.


Functions as a resource to referring providers and referral sources and coordinates care, treatment and services.


Identifies high risk situations and takes appropriate action according to standard operating procedure.


Performs other duties as assigned or needed to meet the needs of the department/organization.


  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader.
    • The use of all safety equipment and personal protective equipment.
