Assertive Community Treatment (ACT) is a multidisciplinary intensive community based support program for people in recovery from mental illness. RHD ACT teams bring together counselors, psychiatrists, registered nurses, case managers and vocational specialists to help individuals with serious persistent mental health challenges maintain employment and social relationships, live independently, and manage their health. The function of the Registered Nurse (RN) is to participate as a full team member and ensure medical needs, physical health issues, and medication issues are integrated into treatment.

RHD is hiring a full-time Registered Nurse to provide community based services to individuals served by the program. This position will also support the operation of the program by participating in the rotating on-call schedule.


Responsibilities/Duties

  • Administer medication and injections per psychiatric provider's order.
  • Conduct health assessments for all clients as assigned.
  • Record injection medication on the mediation administration record.
  • Inform team members of any needs consumers have for medication or laboratory work.
  • Provide face-to-face services in the consumer's home, in public places, wherever necessary.
  • Provide on-call coverage as part of staff rotation.
  • Provide medical education to consumers.
  • Engage in collateral consultation and service coordination planning meetings.
  • Submit accurate and on-time chart documentation and reports.
  • Document telephone and face-to-face contacts with consumers.
  • Document all collateral contacts and community-based activities.
  • Maintain up-to-date consumer charts, statistical forms; and other information and documentation as required.

Required Experience

  • Prefer at least 1 year of prior experience working with persons with severe mental illness and substance use/abuse issues

Required Education

  • Associates Degree in Nursing required

Required Qualifications

  • Must be a Licensed Registered Nurse
  • Valid Iowa Driver License
  • The ability to make ethical clinical decisions in demanding situations
  • The ability to interact professionally with clients, their families and other professionals
  • Organizational skills; ability to complete documentation timely and accurately

Program Summary

The Assertive Community Treatment (ACT) is a mobile service designed to provide intensive supports in the home and community to indivdiuals living with mental illness. ACT is available 24 hours a day, seven days per week, and is prepared to carry out a full range of treatment functions in the participants chosen community. The ACT team consists of the following members: master's level mental health clinicians, registered nurse, substance abuse specialist, vocational specialist, psychiatrist/ARNP, peer support specialist, and a program assistant. Team members work together with the individuals served to promote symptom stability and appropriate use of medication; restore personal, community living and social skills; promote and maintain physical health; establish access to entitlements, housing, work and social opportunities; and help maintain the highest possible level of functioning in the community.
