Registered Nurse, Home Care

Saint Francis Healthcare is looking for experienced Full-time Registered Nurses to join our growing Home Care team!

The Home Care RN will provide case management and skilled nursing care to Home Care patients in their residence and document and coordinate their Home Care services with the patient centered Interdisciplinary Team. Services are provided to adolescents, adults and geriatric patients. The RN is responsible to incorporate Service Excellence standards into job performance.

Schedule: Full-time, Monday - Friday / 6 patients a day / One weekend a month

Job Requirements:

• Licensed to operate a motor vehicle
• Excellent assessment and case management skills
• Interpersonal, organizational and documentation skills.
• Ability to work well in an interdisciplinary team setting
• 1-2 years nursing experience. Home care highly preferred
• Graduate of an accredited nursing program; BSN preferred
• Current licensure as an RN in Delaware
