Job Description
Overview:Position #: 7720Responsibilities:
  • Follows up on internal resources to determine the outcome of interventions completed by other departments. Mobilizes resources in complex cases to maximize patients control and participation over own recovery.
  • Provides support for pain management, pharmacological, and non-pharmacological measures. Assists patient and family in identifying individualized comfort measures.
  • Provides information to patients with validation through documentation and observation. Allows patients and their families the opportunity to ask questions and provide them with accurate answers. Provides patient and family with accurate information regarding health problems enabling them to become active participants in health care decisions.
  • Initiates plan of care based on Capital Health core mission principles. Includes illness, pain, fear, anxiety and any other stressful emotion in plan of care. Advocates for patient when condition prohibits self-advocacy.Incorporates goal setting and consideration of alternatives in the development of a viable plan of care.
  • Independently develops an individualized teaching plan and incorporates aspects of cultural diversity specific to the patient. Confers with treatment team regarding special needs of patient and family upon discharge.
  • Performs and documents an initial history and assessment which reflects the patient's potential for wellness. Anticipates deterioration before the evidence is convincing in terms of changes in vital signs and measurable outcomes, plans for intervention, documents accurately, and evaluates the effectiveness of treatment. Anticipates problems that might arise in complex or unusual clinical situations.
  • Anticipates deterioration while recognizing the importance of reacting quickly and appropriately without overreacting. Thoroughly and accurately documents clinical changes, interventions, and outcomes. Integrates assessment and diagnostic information with intuition to foresee potential age-specific healthcare needs.
  • Independently contacts LIP to seek out anticipated medications and interventions related to changes in patient's condition. Ensures handoff communication includes a report in terms of the situations most likely to develop and the problems awaiting resolution in an established timely fashion.
  • Grasps intuitively the problem and oversees a clinical situation by mobilizing available resources. Displays the ability to rapidly grasp the threatening emergency and initiates treatment measures. Initiates SCTU standing order protocols specific to the patient's condition. Makes medical command to give updates to the appropriate Physician and obtain further orders and instructions as needed.
  • Administers medications. Documents patient's records according to the nursing process, ANA Code for Nurses, and Capital Health Patient Services Division Standards of Practice. Performs, teaches, and demonstrates technical skills specific to assigned nursing unit. Proactively identifies issues to be resolved related to patient education for medication management.
  • Assists unit nurse manager and MICU Clinical Department with the development and revision of the unit based orientation plan and quality assurance (QA). Participates in various aspects of unit based chart review and patient services performance improvement activities related to documentation.
  • Uses discretionary judgments to appropriately modify patient care regimens. Facilitates appropriate response from other health team members using clear and convincing rationale. Obtains new LIP prescriptions to appropriately reflect patient care regimens. Evaluates the quality and appropriateness of care, identifies problems, and initiates and documents improvement of care. Attends unit-based performance improvement meetings, identifies important aspects of care, and responds to the changing needs of the unit.
  • Advocates to LIP and other health members on behalf of patient using clear and convincing rationale to lead the response and treatment needed to meet the patients needs. Obtains orders for modalities and medications in anticipation of changes in patient's current or potential condition prior to transporting a patient. Obtains medications (if not stock of the SCTU) which may or will be used during a SCTU transport three times what a normal transport would take.
  • Participates in the evaluation of unit standards of care and identification of actual or potential problems. Obtains proper equipment and staffing needed to safely transport a patient for the most optimal outcome.
  • Organizes unit activities to mobilize resources in a changing environment. Displays competency in charge and preceptor roles. Follows established policies and procedures for attendance, punctuality, and continuing education. Displays ability to see the entire unit and is self motivated to develop a plan for adjusting staffing patterns and unit changes and needs.
  • Attends advanced preceptor workshop. Utilizes non-productive time efficiently and has not received any disciplinary action related to non-productive time. Displays participation in unit and hospital committees.
  • Displays commitment to Capital Health. Performs other duties as assigned. Possesses a willingness to adapt to changing department demands. Displays flexibility and willingness to assist other units and departments within the EMS structure including BLS, BLS transports, ALS, Education, Clinical, and LifeComm. Performs the duties, roles, and responsibilities of an EMT-P as described in the job standards and performance expectations outlined in the job description

Requirements:
  • Associate's degree or graduation from an accredited school of nursing. BSN preferred
  • One year of full time nursing care performing advanced clinical skills in an acute care hospital critical care unit, emergency department, or SCTU.
  • Demonstrates competency in endotracheal intubations and maintaining an airway using various types of airways (i.e. BVM, OPA, NPA, combitubes, LMA's). Demonstrates competency in all the biomedical equipment used on the SCTU.
  • Valid NJ RN License.
  • Certifications in CPR BLS, ACLS,and PALS.
  • Certification in NRP, PHTLS or ITLS within six months of hire to obtain MICN endorsement.
  • Certification as an EMT-P or certification as an EMT-B.
