The Support Specialist is primarily responsible for facilitating the psychoeducation courses, to include preparation, coordination, and data input into the respective system. This individual will also be responsible for conducting trafficking screening tools on all client referrals and record the information accurately while effectively collaborating with the therapist.
This position works in close collaboration with the program therapist in relation to major job functionality.

ESSENTIAL JOB RESPONSIBILITIES

  • Conduct trafficking screening tools on all client referrals and record the information accurately while effectively collaborating with the therapist;
  • Facilitate the psycho-education courses, to include preparation, coordination, and data input into the respective system;
  • Assist with recommendation for Plan of Care in collaboration with the program therapist;
  • Develop individual rehabilitation plans for and with clients related to daily living functions in accordance with program expectations;
  • Assist with the pre and post outcome measurements in collaboration with program therapist;
  • Collaborate with the external BI Case Managers for referral follow-up and opportunities;
  • Maintain up-to-date client notes in the respective case management platform on a daily basis;
  • Notate the service needs and on-going service needs for all clients, while reporting on these measures weekly to the national program coordinator;
  • Report on outcome measures to the national program coordinator on a weekly basis, as requested;
  • Maintain a current awareness of resources and support services in the local communities;
  • Stay abreast of all agency, federal, and state regulatory requirements related to social services;
  • Attend internal and external trainings, and apply newly gained knowledge on the job;
  • Complete other duties as assigned.

Qualifications:

  • Bachelor’s degree in Human Services, Social Work, or related field of study from an accredited college;
  • At least two (2) years of experience in a similar role;
  • Bi-lingual required (Spanish, Portuguese, Haitian and/or Creole);
  • Prior experience working with refugee and/or immigrant populations preferred;
  • Excellent verbal and written communication skills;
  • Demonstrated conflict resolution and individual engagement skills;
  • Must have an ability to provide excellent and compassionate customer service;
  • Ability to work independently and exercise a high level of confidentiality;
  • Computer and typing skills sufficient to perform essential job functions;
  • Must possess or are eligible to obtain the respective Department of Homeland Security(DHS) clearance approval;
  • Active motor vehicle license and a reliable vehicle with adequate insurance coverage;
  • Pass a criminal history screen, including state and local child protection agency registries;
  • Subscription to and integration of the agency Statement of Faith, Mission Statement, and Cultural Diversity Commitment.
