Job Description:

Job Qualifications: Bachelor’s degree from a four-year college or university preferred; preferably in psychology, social work, or a related field. Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree. Personal experience with recovery from substances is helpful, but not required.

Summary of Job: This position will be part of a Substance Abuse Mental Health Services Administration (SAMHSA) 5 year grant program called Treatment for Individuals Experiencing Homelessness (TIEH). This program will cover the following Indiana counties: Monroe, Brown, Morgan, Bartholomew, Jackson, Jennings, Decatur, Rush, Fayette, and Wayne. The position will work directly with adult individuals with Serious Mental Illness (SMI) or Co-Occurring disorders (COD) experiencing homelessness.

The Outreach/Housing Specialist (OS) will adhere to SAMHSA’s TIP 55: Behavioral Health Services for People Who Are Homeless, beginning with outreach and engagement. To identify/enroll clients, OS will conduct outreach in shelters, encampments, and other venues frequented by people experiencing homelessness. Using evidence-based Motivational Interviewing (MI) and other engagement strategies, the OS will initiate contact with prospective clients, establish trust and rapport, and provide information about the programs services. Outreach will focus on building relationships and identifying/addressing basic needs (e.g., food, shelter, clothing, health problems). OS will conduct limited “in-reach” to incarcerated individuals when appropriate. OS will maintain contact with community organizations/ stakeholders who have agreed to participate in a referral network. Finally, the OS will provide referrals for services and housing with the ultimate goal of securing permanent housing for enrolled individuals.

Essential Duties and Responsibilities:

Supervisory Responsibilities for: This title does not supervise others.

Facilitates client’s use of family, social and community resources; acting as an advocate with external resources and agencies and self-help programs.

Facilitate individual and group interventions covering psycho-educational, addictions, daily living skills and activity therapy.

Develop, implement and monitor clinically and fiscally responsible individualized treatment/discharge plans.

Participate in treatment team meetings for the purpose of summarizing, reviewing and developing client’s treatment goals, progress and use of services.

Maintain necessary communications and reporting with referral sources and other involved parties as approved by the client; provides testimony in court in regards to client’s behavior and condition when requested.

Regular and reliable attendance is required.

Key Performance Indicators

Meet goals and objectives established by grants management team.

Complete ANSA assessments and reassessments on time.

Complete clinical documentation within two business days of the service provided.

Complete treatment plans and other clinical service authorizations in a timely manner.

Other Duties and Responsibilities:

All other duties as assigned.
