Job Summary: Responsible for coordinating, monitoring, analyzing, and reporting on agency practices and performance as directed by the Sr. VP of Clinical Infrastructure in support of agency compliance, performance improvement, and quality initiatives. Provides support for Director of Quality Improvement and Director of Environment of Care. Scope of work varies and there are frequent deadlines. Essential duties of this position listed below.
DUTIES AND RESPONSIBILITIES:
Demonstrates Competency in the Following Areas:

Gather, compile, analyze, and interpret data to create reports, contracts, and internal needs

Participate in the assessment, design, development, and implementation of continuous quality improvement plans and projects, including collecting and analyzing data to identify issues, needs, trends and/or improvement opportunities

Collect and analyze key performance indicators including but not limited to CCO contract pay-for-performance metrics

Provide support and coordination of agency quality projects and initiatives as identified (i.e., Zero Suicide, Client and Family Satisfaction Surveys, flu shot campaign, etc.)

Develop and manage internal chart audit process for quality assurance of documentation practices and identification of areas of risk, including ongoing development of the business analytics process

Create reports and internal documentation to communicate quality activity findings to inform agency and program action plans

Manage agency translation projects and reimbursement

Act as MOTS coordinator for the agency, participating in meetings and ensuring that the agency is in compliance with state MOTS requirements

Assist with the tracking and documentation for Facilities of required inspections and assist with the monthly Environment of Care Report for the Safety Risk Management Meeting

Assist with the ACORN outcomes measure registration system and Prevention Services tracking. Provide support to users as needed

Maintain Family Handbooks materials with updates requested by program staff

Provide support and note taking for Quality Improvement related activities as needed (Family Advisory Council, Documentation Team, Quality Council, etc.)

Support the Clinical Applications team through coordination, written specs for forms and reports, business analytics, and participation in team meetings as appropriate

Complete all agency and departmental trainings required for the position.

Professional Requirements:

Demonstrated knowledge of the principles of confidentiality and the risks associated with the processing of personal health information

Self-motivated, detail-oriented, and organized

Effective time management skills and ability to multi-task

Intermediate to advanced knowledge of Microsoft Office applications, spreadsheet, and database/software concepts

Demonstrated ability to function as a team player as well as independently while successfully following instructions

Demonstrated ability to swiftly and accurately record, process and organize large volumes of detailed oral and written information, material, and assignments

Demonstrated strong communication skills; written and verbal communication with a variety of individuals at all levels inside and outside the organization

SUPERVISORY RESPONSIBILITIES: None
Education: Preference for candidates with college coursework in subjects related to Human Services or Administrative Clerical. Bachelor’s Degree preferred; HS Graduate required. Preference for candidates with college coursework in subjects related to Human Services, Informatics, or Public Health Administration.
Certificates, Licenses, Registrations: None required but preference will be given to candidates with applicable certifications in Quality or Informatics.
Experience: Minimum 2 years’ experience in a role that involves quality improvement. Preference for candidates with human services experience and/or experience in spreadsheet/data reporting and analysis.
Job Knowledge: Operates within established and accepted office procedures and practices. Makes decisions within established guidelines, organizational policy and procedure, federal, state, and local laws, rules and regulations for the requirements of this job description. Deals with highly sensitive and confidential client information.
Skills: Computer skills including, word processing, spreadsheets, and data collation and analysis; solid experience working in and reporting from data in an electronic health record system, strong organizational ability; customer service skills including written, verbal and phone; team player with ability to work independently and complete projects with moderate/minimal supervision
