Job Description
Minimum Qualifications : A Bachelor’s degree in a related field and three years of experience providing direct patient/client services in a health, human service or community-based organization, or institutional setting; OR an Associate’s degree in a related field and five years of such experience; OR seven years of such experience. A Master’s degree in a related field may substitute for one year of such experience. One year of the qualifying experience must include program management and/or supervision of staff.
The experience may include communicable disease, mental health, or alcohol/substance abuse counseling. School health offices are considered an institutional setting and health care providers, such as private physician’s offices are considered health care settings; AND/OR performing oversight activities in a health regulatory organization to determine compliance with appropriate laws, rules or regulations; AND/OR performing responsibilities as a registered nurse; AND/OR conducting epidemiological research including assisting data collection, conducting patient interviews, abstracting medical records, participating in study design, and developing questionnaires AND/OR data analysis. Research must have involved the study of incidence, spread, AND/OR control of communicable or chronic diseases. Laboratory bench research will not be considered qualifying experience.
Weekend shifts may be required.
This position is located in Bethpage, NY. This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting is not available.
Preferred Qualifications : A Master’s degree in public health, epidemiology or a closely related field; at least two years of supervising staff; experience conducting telephone-based or in person interviews; experience analyzing and presenting data; experience with health education, community outreach, linkage to care, and/or social services.Responsibilities : The Public Health Representative IV, located in Nassau County, will monitor epidemiologic investigation and surveillance activities for involving people with laboratory-confirmed, probable, and suspected diagnoses of COVID-19. The incumbent will also be responsible to monitor the provision of technical assistance and health education to patients in order to interrupt ongoing disease transmission; provide guidance and education to staff; monitor and handle complex COVID positive situations; supervise staff and will perform other appropriate related duties.Conditions of Employment : Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required. Weekend shifts may be required. This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting is not available.
