Job Requirements:
Sentara Health Plans is the health insurance division of Sentara Healthcare doing business as Optima Health and Virginia Premier.
Sentara Health Plans provides health insurance coverage through a full suite of commercial products including consumer-driven, employee-owned and employer-sponsored plans, individual and family health plans, employee assistance plans and plans serving Medicare and Medicaid enrollees.
With more than 30 years’ experience in the insurance business and 20 years’ experience serving Medicaid populations, we offer programs to support members with chronic illnesses, customized wellness programs, and integrated clinical and behavioral health services – all to help our members improve their health.
The Performance Improvement Coordinator serves as a key member of Sentara Health Plans Population Health and Performance Improvement team. This position works collaboratively with internal, aligned team members (SMG, SQCN Medical Group), the Health System and other key stakeholders to: facilitate the health plan achievement of the Quadruple Aim (i.e. Improved physician experience, improved patient outcomes, Lower cost of care and improved member experience), by facilitating the development and implementation of health plan level measure improvement interventions and strategies through collaboration with key clinical, medical, health plan team members and other key stakeholders.
Education Level
RN-Bachelor's Level Degree - NURSING
Experience
Required: Health Insurance Industry - 5 years, Healthcare - 5 years

Facilitates, organizes and coordinates plan-level quality measures improvement projects. Evaluation of improvement initiatives. Registered Nurse - professional license or certification are required to hold and maintain such.
