Job Description
Borinquen Medical Centers is based in Miami, FL and is seeking to hire a full-time Peer Educator. The Peer Educator is to provide “peer support” including client orientation and education about health and social service delivery. Recruits participants into testing and treatment programs.


Tasks and Responsibilities

  • Enter, enroll and process all new intakes into Medical Management.
  • Enter, enroll and process all new intakes into SDIS.
  • Design new charts with require documentation according with Ryan White Requirements (RWR) and BHCC.
  • Coordinate and maintain a schedule for patient appointments.
  • Monitor by phone or by mail every day the appointments to follow up with those that missed the appointments.
  • Maintain a schedule in the calendar Re-Assessments according with RWG.
  • Prepare and maintain a patient chart with the necessary documentary of all services provided to or in behalf of a client.
  • Gives out HIV/BHV/STD risk reduction information.
  • Monitors and follows up on service delivery.
  • Counsel newly diagnosed HIV persons in treatment and service options.
  • Coordinate home visits and reaches out to persons/patients that present an adherence problem to treatment.
  • Provides HIV pre and post counseling and testing.
  • Ensure data collection on a timely, accurate and consistent manner.
  • Distributes preventive materials including safe sex kits.
  • Participate in all meetings and training necessary to perform the job.
  • Support staff in assigned project based work.
  • Other duties as assigned by immediate supervisor as required.

REQUIREMENTS

  • High School degree, or equivalent and/or outreach experience.
  • HIV positive.
  • Basic knowledge of HIV/AIDS services and training on HIV funding streams.
  • Bilingual (English and Spanish preferred ; Creole).
