Senior Care Partners PACE, is a Program of All-Inclusive Care for the Elderly (P.A.C.E.) model of care. P.A.C.E. is an innovative program in which we assist elders to live as independently as possible with the team support of health care professionals. P.A.C.E. provides health care services for frail elderly individuals who are eligible for nursing home level of care in the comfort of their own home in the community. Senior Care Partners P.A.C.E. coordinates and provides all needed preventative, primary, acute and long term care services. Primary Purpose

Provide participants care within their home, community setting and/or day center as directed by your leader or designee in accordance with the participant’s assessment and plan of care.

Duties and ResponsibilitiesPrior to being eligible to perform this job, all PCA’s will have training provided by Senior Care Partners PACE in all of the duties and responsibilities outlined below. PCA’s will demonstrate competence in performing the necessary skills and only perform those skills outlined on the plan of care.
  • Provides personal care services for ADLs (dressing, bathing, grooming, toileting, transfers, etc.) and IADLs (shopping, laundry, meal preparation housekeeping, medication reminders, etc.) in participant’s homes and/or in the day center that are essential for their health and independence.
  • Will demonstrate competence in performing the necessary skills and will perform those skills according to the plan of care.
  • Will observe, record in medical record and report changes in participant’s emotional, behavioral and physical condition to appropriate disciplines and/or leader.
  • Encourages each participant’s autonomy and dignity by communicating in a respectful and sensitive manner according to their needs.
  • Promotes participants to engage at maximum level of independence during care.
  • Must care, support and collaborate with everyone involved in the participant’s care including staff and the participant’s family using the participant centered care model.
  • Assists with lifting, turning, moving, positioning and transporting participants into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. as needed, utilizing good body mechanics, proper use of adaptive and lift equipment and adhering to safety precautions.
  • Collaborates with other departments to help collect specimens, record weights, monitor vitals, and/or blood sugars, etc. as ordered in participant’s record.
  • Assists with meal service and clean-up of the dining areas when assigned to the Day Center. Monitors and reports signs of choking or eating problems.
  • Assists with transporting of participants to the clinic area, recreational area, rehabilitation, etc. as appropriate.
  • Participates in activity and social programs with the participants, including 1-1 interactions with participants.
  • Maintains order of personal care areas in the Day Center. Assists with stocking and maintaining personal care supplies for use at Day Center. Notifies PCA Coordinator of low inventory of supplies.
  • Monitors Day Center participants on oxygen tanks and concentrators.
  • Performs assigned tasks while in the Day Center such as laundry, cleaning bathrooms, functional maintenance of equipment, etc.
  • Will assist with escort and outing attendance as directed by PCA Coordinator.
  • May be required to participate in the Extended Hours Program as needed.
  • Participates in On-Call duties as assigned.
  • May be required to assist in training new PCA’s.
  • Utilize the Entrepreneurial Operating System (‘EOS Worldwide” by Gino Wickman) business model to drive focus within the organization.
  • Documents in the participant’s medical record as required by policy.
  • Responsible for completing all assigned company education and file requirements on time.
  • Participates in Quality Improvement activities.
  • Keeps leader informed of problems or issues.
  • May provide student oversight.
  • Other duties as assigned.
Qualifications
  • Must possess, as a minimum, a high school diploma, or equivalent.
  • Must be a Certified Nursing Assistant or complete PCA training that is provided by Senior Care Partners PACE.
  • Well-developed ability to show discretion and maintain confidentiality.
  • Good communication skills, both written and verbal.
  • Detail oriented with good problem solving skills.
  • Basic computer skills.
  • Minimum of one year of experience working with the frail, elderly, or long term care population.
Requirements

  • Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners PACE.
  • Must possess current ServSafe certification or willing and able to obtain as offered by Senior Care Partners PACE.
  • Have the ability to participate in the Participant Care Assistant Training through Senior Care Partners PACE.
  • Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver. (Hepatitis B, TB, PPD specifically and Flu)
  • Must be knowledgeable of current nursing assistant practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult participants.
  • Must have the ability to get along harmoniously with others in a team environment.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Work Environment
  • Has a visible presence while working in the center and is subject to frequent interruptions.
  • Communicates with staff, participants, caregivers, and other organizations as necessary.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
  • May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
  • May work in an unpredictable home environment with variations of cleanliness, temperature, behaviors, smoke, animals, and other potential hazards.
  • Required to drive a car in all types of weather.
Physical Demands
  • Must be able to move intermittently throughout the workday.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset participants within the Senior Care Partners PACE Program and/or center.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to push, pull, move, sit or stand for long periods of time.
  • Lift up to 20 pounds as needed.
  • May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.

You are eligible first of the month following 30 days of service. All salaried staff receive 20 days of PTO accrued during the year, and non-exempt staff receive 18 days, in which you accrue right away. You may start contributing to our 403(b) immediately, and after one year of service the company will match up to 3%. After 90 day of employment you are eligible for a floating holiday at 8 hours, and receive one each calendar year thereafter. All full-time employees receive the six major paid holidays.
