As a key member of our Home Health Rehab team you will further our mission by:
S erving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating nursing services to patients in their home .
Implement the care plan, including patient and family education, in collaboration with other team members.

For more than 30 years, Community Home Health has delivered excellent home care to patients and families in Fresno County and surrounding areas - helping them make the transition from hospital to home.
As a part of Community Regional Medical Center, we are one of the Valley's largest and most comprehensive programs, offering a full continuum of care, including Home Health and Home Infusion services. Home care provides a welcome alternative to frequent and prolonged hospital stays.
Services provided include:
Home Safety Evaluations
Medication Compliance Education and Monitoring
Disease Management Education and Monitoring
Wound Care
Intravenous Medications and Nutrition
Enteral Nutrition
Ostomy Care and Education
In Home Rehabilitation

In this role some of your key responsibilities would include but are not limited to:
Provides occupational therapy services to patients including assessment, treatment, program planning and implementation; along with related documentation and communication, with objective of enabling patient to achieve optimal functional independence. Qualifications:

Education
- Bachelor's Degree required
- Completion of internship program in Occupational Therapy required
- Working knowledge of standard treatment and evaluation tools required
- 2 years of therapy experience preferred
- OT - State of California Occupational Therapist License required
- DL - Valid California Driver's License and driving record acceptable to Community Medical Centers required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
