Job Description
Overview: This position offers up to a $5,000 sign on bonus*

  • Must have two years of Cardiology experience and make a 2 year commitment to Confluence Health

Position Summary:
The Cardiology Nurse Navigator is accountable for navigation and coordination of care utilizing evidenced-based clinical pathways for cardiology patients along the continuum of care for their cardiac condition from diagnosis through treatment. They will provide longiMantudinal comprehensive nursing care to a designated case of cardiac patients. They will have primary nursing responsibilities for the coordination of care, symptom management and education of patients and families. This position will work in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient.

Practice ManagerResponsibilities:Essential Functions:
  • Manage, develop, implement and evaluate initial patient plan of care following clinical pathways in collaboration with the Cardiology Physician and the health care team, to determine the effective utilization of resources and outcome achievement.
  • Facilitate and/or conduct patient, family, and team conferences to develop and monitor a plan of action as necessary. This includes patient education on the disease, treatment options, procedures and medication regimens and side effects.
  • Ongoing assessment of patients for unmet social needs with appropriate referral and communication to social services, community resources, etc.
  • Determine need for further diagnostic work-up; communicate and coordinate necessary tests or procedures to the patient and the healthcare team.
  • Obtain, review and scan outside patient records for new newly referred cardiology patients.
  • Coordinate simple and complex patient care issues in conjunction with other parts of the healthcare system, working interdepartmentally.
  • Determine timeliness/time appropriateness of appointments.
  • Ensures effective and appropriate utilization of resources and efficient patient throughput along continuum of care.
  • Document interim progress notes reflecting coordination of care/services. Intervene when the clinical pathway is not being followed, either due to patient constraints or ineffective follow-through.
  • Provide triage services and symptom management to patients as needed, in-person, telemedicine or over the phone. Communicate symptomology to appropriate team members.
  • Required to maintain active Basic Life Support (CPR) certification.
  • Other duties as assigned.

  • Current Washington State Nursing License.
  • Current BLS Health Provider and ACLS card.
  • Competent in basic computer skills.
  • 3 years of clinical experience.
  • Ability to work independently and perform role with minimal guidance.

  • Cardiology experience.

Working Conditions:
Work is performed at a work station desk and in patient exam/treatment rooms, where space may be limited and confined. Requires a combination of working independently and also working closely with others. Hours may be irregular at times, overtime may be required. General supervision is provided by the Practice Manager with additional instructions, guidance, and direction by the physicians or providers with whom the RN is working.
