Brentwood Behavioral Healthcare is seeking a dynamic and talented MYPAC WRAPAROUND FACILITATOR - Full Time.


The Wraparound Facilitator ensures the process, and the plan itself, is designed to be culturally competent, strengths based, and organized around family members’ own perceptions of needs, goals, and likelihood of success of specific strategies. Wraparound is an approach to individualized care planning that builds on the collective action of a committed group of family, friends, community, professional, and cross-system supports mobilizing resources and talents from a variety of sources to address the needs of children/participants, with complex mental health challenges, and their families.


The MYPAC Wraparound Facilitator is responsible for facilitation of the process of:

  • Engaging the family
  • Convening and facilitating the team meetings
  • Facilitating the development of the plan
  • Implementing and reviewing the plan
  • Transitioning the youth out of formal wraparound

Qualifications

Preferred: Bachelor’s Degree in Social Work, Counseling, or other Mental Health/Social Services related field; and preferred two years’ experience working with children/youth with Serious Emotional Disabilities in an in-participant, residential, or community-based setting


Minimum: Bachelor’s degree and experience working directly with children/youth and formal/informal systems; and preferred two years’ experience working with children/youth with Serious Emotional Disabilities in an in-participant, residential, or community-based setting
