The Memory Care Medication Assistant is responsible for maintaining a safe and secure delivery of medications and health related items to the residents within the facility. The Medication Assistant assists in developing, coordinating and performing the tasks on the service plan according to the resident’s preferences and abilities. Familiarity and willingness to work with elderly population is essential for this role. Previous experience with medication assistance preferred.

Shift Details: Thursday, Friday and Saturday 6am - 6pm (12 hour shifts)

Memory Care Medication Assistant Job Responsibilities:

  • May be asked to provide assistance to residents in Memory Care, Assisted Living, or both
  • Perform resident care with an awareness of dignity and individuality, with understanding and concern
  • Receive report from the previous shift Lead Medication Assistant
  • Read the Communication Log, 24 Hour Log, Shower Schedule and Service Plan log for information and changes at the beginning of each day. Document in the Communication Log and Progress notes, as appropriate throughout the day
  • Assist residents with activities of daily living, environmental orientation, vital signs, first aide, wound care (if delegated), and communication with outside health care professionals.
  • Demonstrate knowledge of infection control and universal precautions
  • Document care given, adhering to facility policies and procedures
  • Maintain clean, neat, comfortable, safe environment for residents, staff, and visitors including housekeeping duties as assigned. Assure medication room door is locked at all times
  • Report deficiencies/problems observed in resident apartments, clothing/linen and facility to Registered Nurse or Administrator
  • Complete Incident Report form as per protocol for any unusual occurrence
  • Observe and communicate any changes in resident physical or mental status to RN and/or Administrator
  • Be thoroughly familiar with fire, missing persons and other emergency procedures, follow facility policy and procedures detailed in fire and disaster plans
  • Avoid loss, breakage, and waste of supplies and equipment. Maintain a neat and orderly Medication Room
  • Wear name tag at all times
  • Assist residents moving in or out as directed
  • Maintain current resident record for each resident including but not limited to, obtaining current physician orders, clarifying orders, alert and monthly charting
  • Order, store and assist with medications according to facility policy and procedure and current physician orders
  • Maintain, transcribe and document in the Medication Assistance Record according to Policy and Procedure
  • Maintain service plans. May participate in Service Plan reviews
  • Ensure use of appropriate log in/log out of pagers, keys, and walkie-talkies for shifts
  • Maintain acceptable attendance record; notify supervisor if late or unable to work in accordance with personnel policies; follow work schedule
  • Require minimal supervision; is a self-starter
  • Organize and utilize time appropriately; set priorities and accomplish assigned tasks
  • Display mature behavior and attitude in speech and action; demonstrate consistently pleasant demeanor, tone of voice, support the philosophy and approach to care used by the facility
  • Keep resident information confidential; respect resident’s rights; respect privacy and right to self-determination of residents
  • Perform unpleasant tasks with grace: work cooperatively with supervisors and co-workers
  • Abide by established policies and procedures of facility. Look for ways to improve facility functioning
  • Keep work areas neat and clean; help maintain appearance of facility
  • Be able to communicate effectively, verbally, and in writing
  • Have ability to spend long periods of feet and to ambulate quickly
  • Be free of non-treated communicable disease
  • Be accurate, dependable, neat and thorough in assigned tasks
  • Ability and willingness to learn quickly
  • Ability to utilize and apply basic management skills
  • Perform all other duties assigned by administrator
  • Must be able to communicate and function in an interdisciplinary team
  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, and move frequently during working hours
  • Must be able to lift 40 lbs. floor to waist, lift 10 lbs. waist to shoulder, lift and carry 40 lbs., and push/pull 40 lbs
  • Must be able to read, write, speak, and understand the English language
  • Must be able to assist in the evacuation of residents

Memory Care Medication Assistant Educational Requirements:

  • Minimum requirement of high school diploma or equivalent
  • Completes all state educational requirements for the position. Attendance at monthly in-services required for position. Receives and maintains First Aid certification, Heimlich maneuver training and Food Handlers Card
