Job Description


Are you a social worker (MSW) with excellent clinical skills? Are you confident as a healthcare professional and value the opportunity to develop relationships with your co-workers and the patients under your care? As a Medical Social Worker at Home Health Care Solutions, you are responsible for the assessment of social and emotional factors related to the patient's illness, need for adjustment to care and response to treatment. Assessment of the relationship of the patient's nursing and other medical requirements to the patient's individual home environment, psychological/cognitive condition, financial resources and available community resources.

Apply if you are:

  • A licensed Certified Social Worker in the State of Indiana
  • Master's degree holder in Social Work
  • One year of social work experience (post master's)

Qualifications

  • Graduate of an accredited school of social work with a Master's degree
  • Current licensure as a Certified Social Worker
  • One year social work experience (post master's) in health care or related sitting
