Job Description


This position is responsible for leading a team of Case Managers and case management support staff in alignment with the Director, Case Management , as well as contributing to the design, development, implementation, operation and improvement of the Case Management program.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

  • Leads a team of case managers and case management support staff in implementing the strategy vision for Tufts Medical Center and Wellforce System.
  • Is accountable for care transitions, LOS optimization, and care progression.
  • Contributes to the recruitment, selection, and assignment of, as well as directly supervises Case Managers and case management support staff (Case Management Assistants, Adminstrative Staff)
  • Leads a case management staff that is adept and efficient at assertively managing patient cases through the continuum of care to achieve the best patient care outcomes and to achieve hospital clinic and financial outcomes as identified in annual strategic and/or performance improvement plan.
  • Supports the compliance with department budgets. Continuously helps to improve our models based on best practices to meet the needs of the patients, families, team and organization.
  • Fosters empowerment of all members of the team and builds in skills that result in high performing teams. Implements key principles of shared governance including unit based council. Maximizes contributions of all employees.
  • Enhances engagement of staff that results in retention of top talent and increased engagement scores.
  • Determines qualifications and assesses performance of department personnel; directs unit specific orientation and continuing education. Monitor and guide team efforts providing development, coaching, motivation, and encouragement. Facilitates strong working relationships between CPP, HIM (health information Management), Coding, Admitting Patient Financial Services and other hospital departments to promote effective and efficient use of all resources.
  • Contributes to the design and implements reliable processes that support care progression and optimize efficiency in safe patient access and care transitions. Supports the development and maintenance of standard operating procedures.
  • Maintains effective relationships with pre and post hospital entities.
  • May perform other job related duties as requested or required.
  • Clinical and operational oversight including maintaining staffing and scheduling plans to meet departmental objectives.

Qualifications


JOB KNOWLEDGE AND SKILLS:

  • Clinical background, R.N., BSN required Masters level preparation preferred
  • Minimum of three to five years experience in case management, quality assurance, utilization management, hospital reimbursement and captation methodologies necessary.
  • Management experience with history of supervising professional staff.
  • Excellent communication and interpersonal skills; must be able to identify and negotiate boundaries of accountability and authority with multiple disciplines.
  • Independent, proactive in taking initiatives;
  • Ability to implement complex work.

ORGANIZATIONAL RELATIONSHIPS:

  • Reports to Director, Case Management.
  • Occasional contact with Department Chairs, Nursing Leadership and Senior Management Group.

WORKING CONDITIONS:

  • Normal office working conditions.
