Job Description
Overview: JOB SUMMARY: The Licensed Vocational Nurse provides direct and indirect patient care services requiring technical and manual skills, within the scope of practice, that meet the psychosocial, physical and general aspects of care, and meets the communication needs of the patient and family; Performs under the direction and supervision of a Registered Nurse.
EDUCATION, EXPERIENCE, TRAINING

  • Current and valid state license as a Licensed Vocational Nurse.
  • Current BLS (AHA) certificate upon hire and maintain current.
  • Minimum one year experience in an applicable setting preferred.
  • IV certification upon hire

  • DUTIES AND RESPONSIBILITIES

RATING

1.
Maintains accurate collection of specimens with appropriate patient labels before sending to the Lab.
2.
Uses two different patient identifiers consistently when drawing lab, administering medications or performing any procedure on a patient.
3.
Follows all medication policies including the medication reconciliation policy consistently.
4.
Establishes priorities of patient care based on essential needs and available resources of time, equipment, personnel and supplies.
5.
Demonstrates understanding of the roles and functions of other members of the interdisciplinary treatment team.
6.
Collaborates with other members of the health team and manages unit issues and/or concerns effectively at the lowest level possible.
7.
Ensures Crash Cart, defibrillation function have been checked per protocol and after each code, appropriate items are immediately replaced and indicated by signature on crash cart checklist.
8.
Assures that all clinical alarms (i.e., cardiac monitors, vital sign monitors, IV pumps, etc.) are consistently set with the appropriate parameters for the patient and are audible at all times.
9.
Demonstrates empathy and respect for patients by explaining all procedures, providing privacy and allowing patient’s to express emotional needs.

Is in compliance with information contained in the Infection Manual (i.e. OSHA bloodbourne pathogen standards, CDC Guidelines), and demonstrates appropriate use of personal protective equipment (i.e. gloves, gowns, masks, goggles) and hand hygiene techniques.

Gives a thorough, detailed efficient change of shift report to ensure consistency in the plan of care.

Ensures each patient/family is provided a thorough orientation to the room, bed and call light system, the routines within the patient care unit, e.g., the times of meal service, visitor hours, physician/nurse rounds, etc., and how to access the Supervising RN or Charge Nurse if needed.

Detects changes in the patient’s condition and communicates information to the RN and physician in timely manner.

Assists with any special tests or procedures performed on the unit within scope of practice. Uses Universal Precautions "time out" on all patients for any bedside invasive procedures in the unit.
15.
Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e. suction and oxygen immediately available when needed.
16.
Detects and reports medication errors and adverse drug reactions (ADR) according to hospital policy. Safely and accurately administers medications and documents properly on the Medication Administration Record (MAR) in a timely manner. Withholds medications and notifies supervising RN or Charge Nurse when patient condition indicates medication may be harmful.
17.
Completes documentation of narcotics given to the patient on the MAR and narcotic sheet.
18.
Initiates patient/family education in accordance with the discharge plan and in consultation with the RN; Documents appropriately in the electronic medical record (EMR) or patient medical record. Initiates discharge and patient / family education planning with proper documentation on the patient record in Medi-Tech.

Complies with all regulatory requirements for clinical pathways, clinical assessments, interventions and documentation, as appropriate

Attends and maintains mandatory training in safe patient handling, trained in safe lifting techniques, includes but not limited to 1) Appropriate use of lifting devices and equipment, 2) Five areas of body exposure: vertical, lateral, bariatric repositioning and ambulation 3) Use of lifting devices to handle patients safely.
21.
Utilizes the National Patient Safety Goals in the work place as related to job performance. Demonstrates awareness of a safe environment for staff, patients, visitors, or any others on the unit.

Performs all other duties as assigned or required.
