Job Description

Licensed Vocational Nurse will provide nursing care to Central Health Plan members enrolled in the Special Needs Plan (SNP)

Duties and Responsibilities:

Licensed Vocational Nurse is responsible for the following:

  • provide nursing care to members receiving health care in an affiliated health care system
  • Collaborate with the nurse practitioner, registered nurse, primary care physician, social worker and the interdisciplinary team in the evaluation and implementation of care plan for members enrolled in the SNP
  • Provide education to members and/or their caregivers in the management of their health conditions
  • Provide information to members and/or their caregivers on community resources
  • Attend all appropriate staff meetings (interdisciplinary team meetings, nursing staff meetings, other committees as appropriate or as assigned)

The LVN is responsible for:

  • Contacting members and/or their caregivers, with verbal consent from the member to discuss the Interdisciplinary Care Plan (ICP) and revise the ICP as needed in accordance with members identified needs and goals.
  • Care plan discussion will include, but not limited to: health education related to health promotion, disease management, geriatric conditions, medications and recommended therapies or interventions, self-management of health conditions, and assistance with obtaining appointments and/or services and referrals to community resources.
  • Documenting any changes in the member’s health status, including all interventions in the member’s record found in the care plan system.
  • Assessing and intervening in any medical or psychiatric crises presented by the member during ICP discussion, such as, but not limited to referral to PCP, SW, provision of 24 Hour Suicide Hotline number or calling the Psychiatric Mobile Response Team as needed.
  • Inviting members and/or their caregivers to participate in the Interdisciplinary Care Team (ICT) meeting and identify necessary accommodations needed by members such as, but not limited to translator.
  • Identifying specialists that are involved in member’s care and notify the SNP coordinator to include them in the ICT invite sent to health care providers.
  • Identifying and requesting needed records (i.e. labs, progress notes/reports) for the SNP coordinator to assist in obtaining from PCP’s or specialist’s office.
  • Providing thorough and concise report regarding members identified health care needs during the ICT meeting.
  • Completing and sending the PCP Recommendation form as needed.
  • Implementing the ICP as initially approved or with recommended changes.
  • Maintaining telephonic contact with the member to follow up on the implementation of the ICP based on target dates assigned to short and long term goals and completing the process as outlined in the ICP policy and procedure (P&P).
  • Creating a call log for each telephonic encounter with members.
  • May conduct follow up in-home assessment for members enrolled in the GRACE Program.

Administrative duties:

  • Attend required meetings.
  • Perform other duties, projects, and actions as assigned.
  • Assist in monitoring call logs to ensure that ICP discussion meets guidelines as outlined in the ICP P&P.

Qualifications

  • Bilingual in Chinese-Mandarin/Cantonese required
  • Graduate from a program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
  • Current California State License
  • Current BLS Certificate
  • Able to communicate and work well with team members
  • Have good customer service skills
  • Be professional and have good work ethic
