Job Description
Full-time with Minimum On-Call Rotation

Coverage Areas: Porter, La Porte, Starke, Jasper and Lake Counties, mileage paid at the IRS rate.

$2,000 bonus

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

Functions
  • Assists the RN and/or physician in performing specialized duties related to end-of-life care.
  • Implements and continues the appropriate teaching in cooperation with the Registered Nurse.
  • Observes the patient for changes in their condition and reports these changes to the appropriate physician and registered nurse.
  • Records visits to patients per agency policy, including other pertinent information about patient and family.
  • Follows care plans as developed by the Registered Nurse and contributes data to the RN in the development of the plan of care and assists in carrying out the plan.
Qualifications
  • License and experience.
  • Graduate of a state accredited school of practical nursing.
  • Must hold and maintain current LPN license in the State of Indiana.
  • Evidence of experience in related health care field.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.
