JOB SUMMARY

The Licensed Vocational Nurse (LVN)/Licensed Psychiatric Technician (LPT) reports to the Assistant Program Director and works as part of a treatment team in achieving the goals and objectives of the Diversion program to provide quality care for participants. The LVN/LPT will provide direct nursing support to program clients. The LVN/LPT will provide education to the Treatment Team, clients, and to families of clients (as appropriate) in many health areas, with special attention given to psychotropic medications.

ESSENTIAL JOB RESPONSIBILITIES

  • Provide high-quality nursing services to clients living with Severe Mental Illness (SMI) and dual-diagnosis. Services include but are not limited to medication education and support (scheduled, walk-in, individual, groups, and crisis intervention), consultation with Psychiatrist and physicians regarding clients and medication adjustments, facilitation of blood draw for labs as ordered, assistance with medical treatments/procedures as prescribed.
  • Per authority of license, chart appropriate records, review medicine orders regularly, obtain appropriate physician approval, and coordinate with pharmacy as needed.
  • Observe client’s physical condition and behavior, report significant changes to professional staff, and record progress notes on clients.
  • Responsible for the monitoring and observation of client medications; administration of prescribed injections; and tracking of prescribed medication and dispensing as appropriate.
  • Transport clients to doctors’ appointments and laboratories for tests.
  • Participate in carrying out individual and group treatment programs for clients as part of the treatment team and encourage client participation in treatment activities which will include visiting clients in their homes, board and care facilities, or in other community placement locations to carry out prescribed treatment plans.
  • Provide services to enhance clients’ development of self-reliance in daily living and independent living skills.
  • Order medical supplies and check delivery; stock and maintain inventory.
  • Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  • Adhere to reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.
  • Prepare and maintain a variety of records, and prepare reports including documenting services to meet all clinical program and state requirements, including Short-Doyle/Medi-Cal requirements.
  • Provide the Assistant Program Director and Program Director with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  • Consistently meet LVN/LPT program performance standards including productivity, sign and submit documentation goals, and attainment of 40 hours of job related in-service training per year.
  • Attend daily team check-in meetings, weekly one-on-one meetings with the Supervisor, and critical bi-monthly meetings including staff meetings, and high risk resource team (HRRT) meetings. Provide effective and thorough feedback regarding client health and medication engagement, progress, barriers, and needs.
  • Exercise sound judgment when providing client services, prioritize safety, manage time efficiently to meet deadlines, and respond quickly/effectively in emergencies or pressured situations to support de-escalation/Pro-Act skills.
  • Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, infection control and preventing potential safety risks for staff, clients and others.
  • Will drive on company business and transport clients as needed.
  • Follow Turning Point’s attendance/punctuality, paid time off (PTO), meal and rest break period policies and procedures.
  • Model professional employee behavior including punctuality and regular attendance. Report to work as scheduled, on time and prepared to begin work upon arrival. Remain at work throughout the entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

ESSENTIAL JOB REQUIREMENTS

  • Possession of a valid license as a Licensed Vocational Nurse or Psychiatric Technician as issued by the State of California.
  • Any combination of experience and education that could likely provide the required knowledge, skills and abilities is qualifying. A typical way to obtain the knowledge, skills and abilities would be: Supplemental college course work in behavioral or social sciences, and one year of experience performing duties comparable to a Mental Health Worker or related nursing experience.
  • Knowledge of basic concepts of mental illness and mental health including the physical, emotional and social needs of emotionally disturbed individuals.
  • Knowledge of fundamentals of psychiatric and mental health nursing care.
  • Knowledge of basic medical and psychiatric terminology.
  • Knowledge of principles and techniques involved in the care and treatment of individuals or groups of mentally ill, emotionally disturbed, or mentally retarded clients.
  • Knowledge of contra-indications, side effects and signs of medication complications.
  • Ability to apply routine psychiatric/medical nursing care to clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals
  • Recognize symptoms indicative of adverse reactions.
  • Communicate effectively in written and spoken English.
  • Ability to communicate effectively in written and spoken Spanish and/or Hmong is preferred.
  • Knowledge of record keeping procedures related to client charts.
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint.
  • Ability to operate within Electronic Medical Records (EMR) system.
  • Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment physical, drug screen, general physical, and TB test. Hepatitis B Immunization Consent-Refusal form must be completed. Hepatitis B Immunization is available at the request of the employee.
  • Must maintain good relationships with consumers, co-workers, government and civic representatives, as well as community members within whom the Agency is transacting business, and relate to them in a professional manner.
  • Must be available to work all shifts as needed.
