The Primary Care integrated care teams (ICT) consist of a Provider, a RN Case Manager, a Certified Medical Assistant/LPN and an ICT Coordinator. This integrated team has the opportunity to build relationship with patients over time to address their preventive, acute and chronic health care needs which results in a high continuity of care and interdisciplinary team work.
This position is responsible for all clerical duties, scheduling tasks and support of team Case Manager as assigned. Each Integrated Care Team will serve a population of approximately 900 – 1, 200 patients. This position will work closely with the Case Managers in order to meet the daily demand of patient requests and needs that occur outside of a provider visit.
This position requires the ability to answer, screen and assist with sensitive patient phone calls. This position requires close, collaborative interfacing among all members of the Integrated Care Team. This position requires excellent organizational and prioritizing skills to work in a fast paced environment supporting all team members in coordinating care of the patient population.Responsibilities::
  • Answers incoming phone calls for the ICT.
  • Demonstrates exceptional phone etiquette.
  • Directs phone calls to appropriate care provider.
  • Transcribes and relays messages that are clear and concise within Cerner and/or handwritten format.
  • Verbally communicates detailed information clearly, concisely, assertively and in a timely manner to the team.
  • Maintain voicemail messaging system with proper delegation of patient needs/tasks.
  • Send/receive faxes and distribute timely to the appropriate team member.
  • Mail correspondents as directed by the team.
  • Utilize EHR to communicate with team members
  • Interacts, communicates and collaborates with team members in order to maximize group effectiveness.
  • Maintains ICT patient scheduling – care of schedule to assure same day slots, scheduling of patients and all related scheduling tasks for appropriate patient population.
  • Prioritizes work to assure deadlines are met and facilitates team/patient flow.
  • Maintains confidentiality following SEARHC policy and HIPAA Regulations.
  • Assure paper/supplies are stocked for team.
  • Assist with chart reviews as requested by the case manager.
  • Manages patient referrals to outside agencies.
  • Collaborates with HIM to: request, document and scan patient health information between team and outside sources.
  • Facilitates patients to properly file appropriate healthcare documents and paperwork.
  • Convey healthcare information to patients in accordance with HIPAA regulations as deemed appropriate by the case manager/provider.
  • Performs other tasks as delegated by the case manager in an effort to balance work flow processes among the team for greater efficiency in meeting patient population care needs.
Qualifications::

Education/Experience:

  • High School Diploma or equivalent
  • Two (2) years general office experience
  • Two (2) years medical office experience preferred.
  • Healthcare related course work may be exchanged for 4 months of experience per semester – examples include, but not limited to Medical Terminology, Anatomy/Physiology or Medical Coding.

License/Certification:

  • Medical Assistant Certification (CMA) can substitute for general office experience.
  • Certified Nursing Assistant can substitute for 1.5 years of general office experience.
Knowledge, Skills and Abilities:

Knowledge

  • Knowledge of electronic appointment management
  • Knowledge of multiline phones/fax machines and office equipment
