Now Hiring - Intake Coordinator - Puerto Rico


Hours: 8am - 5pm


The Intake Coordinator is responsible for, but not limited to:

  • Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
  • Insures that whenever possible, all referrals are converted to admissions
  • If referrals do not convert to admissions, the Administrator will be notified
  • Track all inquiries and referrals daily. This report will be given to the Administrator weekly
  • Assists patients, families and responsible parties in completing the admissions packet as needed
  • Works collaboratively with appropriate staff to insure written or verbal consents are obtained
Qualifications:

Education:

Associates Degree in a related field


Professional Experience:

  • Preferably 5-8 years
  • Experience in a related capacity
  • Must have a Valid Driver's License and reliable source of transportation
  • Must be willing to travel
  • Excellent communication, interpersonal and organizational skills
