Job Overview
Develops relationships with case managers, social workers and nurses within St. Joseph's Healthcare System for sub-acute, long term care, hospice and respite referrals to St. Vincent's. Reviews medical records of patients pending discharge for possible post acute acute admission. Meets with patients and their families to promote St. Vincent's services and care and assists with admission paperwork.Qualifications: Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's degree in Social Work or closely related field. Bachelor's degree in Nursing preferred. New Jersey Certification as a Social Worker OR a Master's degree in Social Work and licensure as a Cerfified Social Worker OR licensure as a Registered Nurse by the State of New Jersey required. Three to five years of long term care and acute experience required. Excellent interpersonal & communication skills required. Attention to detail & organized.
