Job Description

Interim HealthCare of West Michigan is seeking a part-time Nurse Practitioner to join our dedicated team of Hospice professionals. As a Hospice NP you will come along side our clinician team, contributing your leadership and support while focusing on face-to-face assessments.

The Hospice Nurse Practitioner will help lead the charge in coordinating and managing client care plans with specific knowledge in home health, hospice, bedside care, symptom management, and crisis and family intervention.

Hospice Nurse Practitioner Essential Functions

  • Complete initial/comprehensive assessment of the patient/family to determine hospice needs
  • Complete ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family
  • Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician
  • Make necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change
  • Actively participates in IDG meetings
  • Documents an individual patient's progression in end-stage disease in the patient/family record as appropriate
  • Provides assistance to patients and families in goal setting and care plan development with end-of-life discussions
  • Plans care utilizing community resources and act as a liaison by relaying information to other health care professionals and agencies to provide for continuity of care
  • Completes other assignments as requested and assigned

Hospice Nurse Practitioner Requirements

  • Graduate of an accredited school of nursing with an advanced degree in nursing - either MSN or DNP
  • Licensed to practice as a Nurse Practitioner in the state of Michigan. The license is active and in good standing
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years
  • One (1) year of hospice/palliative care experience preferred
  • Active CPR certification
  • Reliable transportation with a valid state driver's license and current car insurance
