Job Description

JOB PURPOSE:
The Home Health Intake Coordinator receives patient referrals for all branch locations from a variety of sources and methods, reviewing appropriateness of referral in accordance with law and regulation, agency staffing resources and the patient’s care needs in the home care setting. Makes quick decisions regarding acceptance versus rejection of referred patients, responding timely to referral sources. Coordinates transitions of care between referral sources and agency staff, including the Clinical Managers, Financial Services Representatives and clinical staff to assure that timely care and service is provided in accordance with regulation, agency policy and patient needs. Works under the supervision of the Athens Clinical Manager.



Requirements

MINIMUM EDUCATION REQUIRED:
Graduate of a nursing program.

MINIMUM EXPERIENCE REQUIRED:
Minimum one year of nursing or managerial experience in home health setting plus 1-2 years of experience in medical surgical nursing or other related acute care setting.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a Registered Nurse. Valid Georgia Driver’s license.

ADDITIONAL QUALIFICATIONS:
Experience in inside or outside sales or business development a plus. Knowledge of home health regulations as it pertains to acceptance of patients a plus
