Job Description

As the Home Health and Hospice Aide you will provide personal services such as bathing, grooming and assistance with ambulation to skilled home health patients under the direction and supervision of a registered nurse or therapist. Complete personal care interventions according to the plan of care and documents accurately.


Shift Details

Full Time (Monday-Friday) 8:00 a.m. - 4:30 p.m.

MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

  • Provide care for skilled home health patients consistent with the tasks listed on the written plan of care
  • Complete personal hygiene for patients, including bathing (tub, shower, or bed bath), oral hygiene, hair care, shampooing, shaving, skin care, nail care, foot care and dressing
  • Assist client with mobility, including walking, transferring, turning and positioning including the use of transfer device such as the Hoyer Lift and transfer Assist client with prescribed exercises to which the client and the Aide have been trained.
  • Assist with toileting needs, including assistance with bedpan, urinal, bedside commode, toilet use and incontinence care
  • Assist with reminders to take medications that are ordinarily self-administered
  • Measure and record fluid intake and output
  • Observe, report and document observations and changes in patient’s condition and needs to the registered Documents services and observations in the client’s record.
  • Prepare or assist with feeding of meals and snacks; includes clean-up of kitchen area
  • Perform other activities as assigned including but not limited to reading and recording, blood pressure, temperature, pulse and Foley catheter care, basic infection control procedures; emergency procedures and care of the body after death.
  • Comply with agency policies and procedures relative to infection control and safety

PHYSICAL REQUIREMENTS:

  • Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours

REQUIRED KNOWLEDGE, SKILLS AND ABILITY:

  • Certificate to practice as a Certified Nurse Aide in the State of Virginia in good standing
  • Demonstrated ability to read, write, and speak English
  • Current CPR certification for healthcare provider
  • Must possess good verbal and written communication skills
  • Willingness and ability to use personal smart phone for work related communication, documentation, schedule and visit tracking
  • Must be able to meet deadlines, work under limited supervision
  • Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization
  • Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
  • Proof of motor vehicle insurance

REQUIRED EDUCATION AND EXPERIENCE:

  • Two years of nursing assistant experience in community Health (Hospice or Home Health).
  • Completion of classroom and supervised practical training totaling at least 75 hours, with at least 16 hours devoted to supervised practical
  • Completed 12 hours of in-service
  • Completes an annual Skills Check by an RN
