Senior Care Partners PACE, is a Program of All-Inclusive Care for the Elderly (P.A.C.E.) model of care. P.A.C.E. is an innovative program in which we assist elders to live as independently as possible with the team support of health care professionals. P.A.C.E. provides health care services for frail elderly individuals who are eligible for nursing home level of care in the comfort of their own home in the community. Senior Care Partners P.A.C.E. coordinates and provides all needed preventative, primary, acute and long term care services. Primary Purpose
Responsible for oversight of the organizations health management system to include collecting, analyzing and reporting on participants health outcomes. The role of this position is to promote coordinated care and wellness through evidence based decision support in clinical care by managing the health population management system integrating financial and provider data.
Duties and Responsibilities
  • Manages the health population management system, processes and vendor.
  • Analyzes the clinical, financial and operational data to improve participant care, quality and cost savings.
  • Utilizes business intelligence to analyze data presenting actionable information to clinical staff and department leaders.
  • Prepares analytical results and presents to management.
  • Institutes clinical informatics to assist in coordinated care.
  • Oversees and trains on the IT systems used for medical personnel.
  • Oversees electronic health records, manages vendor relations and maximizes functionality.
  • Track and maintain Medicare and Medicaid files.
  • Oversee member enrollment information, benefit coordination and disenrollment processes.
  • Oversee communications with PDAC, Acumen ERpt, Milliman, EDV, RAPS and EDS and other third-party reporting.
  • Oversees data tracking and reporting, such as CMS risk adjustment reporting, encounter reporting and PACE industry benchmarking and other adequate information to contracted third party vendors.
  • Oversees the data and information required to be reported to CMS, HPMS and MDHHS as it pertains to participant care activities and outcomes.
  • Performs analysis of data to identify potential program risks and enhance compliance with all applicable laws, rules and regulations.
  • Oversees collection and maintenance of financial and utilization data for reporting internally and externally.
  • Assists with audits of the HIM, provider audits and Quality Assurance audits.
  • Works with care teams to enhance data collection processes.
  • Collaborate with care providers to ensure accuracy of health information.
  • Works with organizational staff to ensure efficient and timely reporting of information through the use of electronic medical record.
  • Responsible for making sure all aspects of electronic medical records system runs smoothly.
  • Builds and maintains training and development documentation for system processes.
  • Evaluates and implements operational processes and assists with system projects.
  • Ensures the provision of accurate data for improvement efforts.
  • Keeps supervisor informed of problems or issues; performs other duties as assigned.

Qualifications
  • Bachelor’s Degree in Nursing with Health Informatics or Business Management required or equivalent experience preferred. Masters Degree preferred.
  • Minimum of 3 to 5 years of professional and progressively responsible management experience required. Previous experience in a non-profit healthcare setting preferred.
  • Experience should include data analysis, management, compliance, quality and resource development.
  • Familiar with data exchange standards.
  • Detailed oriented with good problem solving skills.
  • Proficiency with MS Office, Work & Excel.
  • Good organizational and project management skills.
  • Strong communication skills, both written and verbal.
  • Demonstrates resourcefulness in setting priorities, proposing new ways of creating efficiencies and guiding investment in people and systems.
  • Must be technologically savvy with the ability to point to examples of having worked with IT staff to develop and implement new processes and systems that increases efficiency in a fast-moving environment.
  • Must possess personal qualities of integrity, credibility, and commitment to Senior Care Partners PACE mission.
Specific Requirements

  • Must have reliable transportation and be able to travel between centers as needed; current valid Michigan driver’s license and current valid vehicle insurance required.

  • Possess current CPR certification by AHA, or willing and able to obtain as offered by Senior Care Partners PACE.
  • Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver. (Hepatitis B, TB, PPD specifically, Flu)
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Possess the ability to collaborate effectively with other IDT members.
  • Must care, support, and collaborate with everyone involved in participant’s family using the patient-family centered care approach.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Work Environment
  • Has a visible presence while working in the center and is subject to frequent interruptions.
  • Communicates with staff, participants, caregivers, and other organizations as necessary.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
  • Required to drive a car in all types of weather.
Physical Demands
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, and have flexibility personal integrity, and the ability to work effectively with participants, staff, and support agencies.
  • Must be able to relate to and work with the ill, disabled, elderly, and emotionally upset participants within the Senior Care Partners PACE program and/or center.
  • Must be able to push, pull, move, sit or stand for long periods of time.
  • Lift up to 20 pounds as needed.
  • May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.
BenefitsWe offer a competitive benefits package including medical, dental, vision, Teledoc, HelpNet, and company paid life insurance and AD&D at $50,000 each, with the option to purchase additional voluntary insurances – STD, LTD, AI, CI, and additional life and AD&D. You are eligible first of the month following 30 days of service. All salaried staff receive 20 days of PTO accrued during the year, and non-exempt staff receive 18 days, in which you accrue right away. You may start contributing to our 403(b) immediately, and after one year of service the company will match up to 3%. After 90 day of employment you are eligible for a floating holiday at 8 hours, and receive one each calendar year thereafter. All full-time employees receive the six major paid holidays.
