Job Description

Responsibilities: As a Physical Therapy Assistant (PTA) at Family Home Health Services you will:
  • Provide therapy services for each patient based on their plan of care under the supervision of a Physical Therapist, within your scope of practice, in the patient’s home.
  • Contribute to the evaluation and planning process that addresses physical, psychosocial, environmental and safety factors while considering physician orders, relevant history and patient’s functionality.
  • Provide patient/caregiver education with a focus on goal and outcome achievement as established in the plan of care, documenting patient response.
  • Assist in evaluation of patient’s functional status and overall abilities to help determine realistic outcomes.
  • Prepare and submit clinical and progress notes, reflecting activities, findings and patient’s reaction to treatment and changes to patient’s condition in a timely manner following visit or communication.
  • Attend virtual staff meetings and participates in patient case conferences/coordination of care communication.
  • Other duties as assigned.
Qualifications:
  • Graduate of an accredited Physical Therapy Assistant Program.
  • Licensed as a Physical Therapy Assistant in Florida.
  • At least one year of experience treating adult patients, preferably geriatric patients.
  • CPR certification.
