Under the general supervision of the Director of Utilization Management, The ED care coordinator performs all functions in an ethical and professional manner. The role of the ED care coordinator potentially includes all the functions within the evolving core of Care Coordination and is individualized to each patient. In addition to the function of access, it is primarily twofold: providing utilization management, care coordination and discharge planning for all patients presenting to the ED in need of care. The care coordinator works in tandem with the ED healthcare team to problem-solve patient difficulties in the care coordination arena. In addition, the ED care coordinator should participate in the orientation of new staff to the ED and serve as a preceptor for care coordination personnel.
Minimum Education: Associates Degree required in nursing.
Professional Licensure/Certification/Registration: Current Virginia State RN license as appropriate to profession required within three months of employment.
Experience: Three years recent health care experience preferred.
Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
