Job Description

The Aliso Ridge Behavioral Health facility is the flagship campus of the Orange County Behavioral Health system. The newly-built facility is located in Aliso Viejo and offers 119 beds, serving children, adolescents, and adults.

The facility is modern, light, and secure. Every aspect of the building is meant to provide an inviting, comfortable, and safe space for both our patients and staff. The goal was to build a world-class facility that matched the high level of care and attention to detail provided to our patients.

Offering both inpatient and outpatient programs, Aliso Ridge Behavioral Health is licensed as an acute psychiatric hospital. The hospital has four units of 22-23 beds for various patient groups, along with a separate 28-bed unit for specialty care. In addition, the following amenities are available at the facility:

  • Fitness equipment and basketball court
  • On-site dining
  • Extracurricular programs

POSITION TITLE: Discharge Planner/Case Manager

REPORTS TO (TITLE): Director of Clinical Services

EMPLOYEE PAY STATUS: Non-Exempt

DESCRIPTION OF POSITION:

The Discharge Planner functions under the direct supervision of the Director of Clinical Services and assumes responsibility for the overall discharge planning needs and placement of the patient. This includes resident needs, aftercare needs, advocacy needs, and entitlement needs.

KEY RESPONSIBILITIES:

  • Review all new admissions and meet with patients admitted from the previous day and complete discharge planning assessment to determine all discharge planning needs as indicated by treatment team.
  • Review case management needs of patient throughout treatment experience and determine placement outcomes.
  • Attend treatment plan meetings and maintain documentation and intervention for treatment planning.
  • Communicate all discharge planning issues with all members of the treatment team.
  • Develop and utilize a discharge resource book identifying community resources, such as Section 8 and other financial resources available in the community for patients.
  • Document all patient contacts in the medical records.
  • Assist patient with phone calls, appointments, and aftercare needs.
  • Educate patients on the need for aftercare placement.
  • Maintain the discharge calendar to assure proper discharge plans are arranged prior to the patient leaving the facility.
  • Coordinate pending discharge plans.

Requirements

Education and Licensing:

  • Bachelor’s Degree in Social Work or other Human Services field required. A Master’s Degree in Human Services field preferred.
  • CPR within thirty (30) days and maintain current.
  • CPI within sixty (60) days and maintain current.

Knowledge and Experience:

  • A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, and communication with external review organizations. Experience in coordinating care with programs and outside referrals.

Skills and Abilities:

  • Knowledge of patient assessment, family motivation and treatment planning techniques.
  • Knowledge of utilization of crisis intervention skills.
  • Knowledge )of patient rights, advanced directives, adult and child abuse laws, and involuntary procedures.
  • Knowledge of complex patient care planning.
  • Excellent leadership and negotiation skills.
  • Excellent organizational and interpersonal communication skills.
  • Skill in interacting with multiple individuals with diversified roles and perspectives.
  • Knowledge of external review organizations (i.e. Managed Care, Medicare).

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:

  • Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.
  • Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population.
