Description:
  • I Accountability Objectives:

    Under the direction of the Manager, Community Pharmacy Services and Manager, Inpatient Pharmacy Services and the Clinical Pharmacy Manager, provides clinical support and distribution services (pharmaceutical care) in conjunction with other members of the healthcare team to ensure high quality, safe and cost effective drug therapy to patients.


    II Position Qualifications:

    Minimum Education, Licensure / Certification and Experience Required.


    A. Education

    • An entry level degree of B.S. in pharmacy from an accredited College of Pharmacy or equivalent is the minimum acceptable level of education.
    • A Doctor/Pharmacy degree and/or additional ACCP accredited certification is strongly preferred.


      B. Licensure / Certification

    • Registered pharmacist possessing or eligible for a Michigan Pharmacist and Controlled Substance license.
    • Current Basic Life Support (BLS) certification.
    • If Pharmacist is a member of RRT or responds to Codes, Advanced Cardiac Life Support (ACLS) certification required.

      C Special Skill / Aptitudes

    • Interpersonal skills necessary to communicate effectively with a wide variety of hospital healthcare personnel and patients.
    • Commitment to adhere to activities that promote the vision and mission of SJM-O and the Department of Pharmacy and the principles of continuous quality improvement within the organization.
    • Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.


      D. Experience

    • Previous hospital, retail or long term care experience depending on departmental function area.

      III Duties / Responsibilities:

    • Interprets and evaluates all physicians' medication orders to ensure appropriate drug therapy for each patient in accordance with legal and regulatory policies, rules and standards.
    • Reviews, maintains, and screens patient medication orders and profiles for allergies, drug-drug interaction, drug-food interactions and therapeutic duplication and out of range doses.
    • Communicates with physicians and nurses and other healthcare providers and patients to intervene on orders that are unclear, incomplete, inappropriate, or could cause drug misadventuring and recommends alternative therapy. Intervention activity is documented appropriately.
    • Provides drug/poison information education programs to healthcare professionals in response to drug information questions or in-service education.
    • Accurately prepares and dispenses medications and IV admixtures to both inpatients and outpatients of SJMO.
    • Compounds and dispenses medications and injectables recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality.
    • Competency must be demonstrated before compounding or checking compounded products.
    • Adheres to USP 797/800 Guidelines.
    • Directly supervises pharmacy technicians and interns in distributive functions and department operations and assists in yearly performance review.
    • Assists in the training and orientation of new pharmacists, pharmacy technicians and pharmacy interns.
    • Assists in training undergraduate and graduate students in operational or clinical areas of responsibility.
    • Maintains control and accountability for disposition of Controlled Substances, dangerous or toxic drugs and investigational drugs as required by Federal and State regulations. Properly maintains all legally required records.
    • Participates in all pharmaceutical care activities such as required including patient rounds, therapeutic drug monitoring, patient teaching or counseling, admission drug histories, pain management, etc., as appropriate.
    • Makes appropriate decisions quickly using good judgement to reduce drug costs and ensure efficacy and safety of drug therapy for the entrusted lives of SJM-O and CHCS,
    • Demonstrates and actively promotes an understanding and commitment to the mission of St. Joseph Mercy Oakland through performing behaviors consistent with the Trinity Health Values.
    • Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as other policies nd procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behaviors.
    • Supports and conducts one's self in a manner consistent with customer service expectations.
    • In accordance with unit or departmental practice, determines that appropriate charges have been entered for the correct patient, encounter, date/time of service, with any required modifiers. Make corrections as needed per charge capture policy/practice.
    • In accordance with unit or departmental practice, actively participates in on-going education and communication regarding revenue management.
    • In accordance with unit or departmental practice, assists with tracking and monitoring of equipment assigned to the unit. Requests services for maintaining equipment as needed.
    • In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment, and care management, performing these activities concurrently with provision of care throughout the shift.
