Community Living Skills Coach – Mental Health

Job Description:

Provides education, training, and support to residential clients to maintain the most independent community living possible. Partners with each client on caseload in pursuing Service Plan goals through coaching, teaching, and counseling within the scope of the program’s services. Collaborates with other East House staff, Rochester Psychiatric Center and other housing providers to coordinate client services. Manages assigned caseload efficiently and effectively to maintain accessibility to each client. Maintains familiarity with all clients through participating in case reviews, reviewing communication systems, etc. Addresses needs of the clients that occur during on-duty shifts. Coordinates with designee in assisting clients with funding and financial matters. Teaches, supports and assists in the daily upkeep of environment (e.g., light housekeeping, cooking, food and supply purchasing, etc.). Teaches, supports and assists clients in their responsibilities for cleaning, meal preparation, and safe and sanitary conditions within the living situation. Provides supervision of clients in managing their medications. Adheres to East House medication policies and procedures. Maintains documentation to the standards defined by regulators, funders and East House policies and procedures. Assists clients to follow through with appointments (e.g. medical, therapy appointments), attend these appointments as needed and assist with treatment recommendations. Informs appropriate providers of outcome.

Hours: 35 hours per week, Tuesday 8 AM to 3:30 PM, Wednesday Noon to 7 PM, Thursday 8 AM to 3:30 PM, Friday Noon to 7 PM and Saturday 8 AM to 3:30 PM

. Requirements:

High school diploma and 1-2 years of related human services experience OR

Associates degree and 1 year related human services experience OR

Bachelor’s degree in human services or related field.

Valid NYS motor vehicle license, safe driving record, and an insured personal vehicle for use at work required.

Compensation:

Wages are determined based on job-related education and experience.
