**Applicants must be willing to work in all 13 counties of District 2 DHP to include some weekends and possibly non-traditional hours to fulfill the essential functions of the position

District 2 Public Health serves 13 counties in northeast Georgia, including Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White counties. Job Responsibilities

Under general supervision, provides a broad range of paraprofessional social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties.
Additional duties may include, but not limited too:
1. Directly reports to the Chronic Disease & Health Promotion Coordinator
2. Collaborates with the Epidemiology Team to identify areas that are severely impacted by COVID-19, with low immunization rates & high morbidity & mortality rates to conduct outreach & engagement activities
3. Collaborates to identify essential services and resources within the targeted areas & to establish a repository of community contacts, partnerships, & resources to connect populations to care., eliminating barriers & serving as an advocate for healthier living
4. Develops a resource guide for the area with information such as housing, food banks, clothes closets, transportation etc. for those vulnerable population.
5. Collaborates with the CDHPC, District & State level staff to create, develop, implement, disseminate & educate on appropriate materials.
6. Serves as a liaison between services & assists with the coordination & transition of those services while building relationships with local agencies.
7. Works closely with the CDHPC & District staff to encourage & promote cultural competence & to advocate for vulnerable populations, identifying barriers or needs for resources & services.
8. Participates in local community health taskforce/coalition meetings, calls with District & State leadersip as directed, the Georgia Community Health Work Network & the Community Health Worker Advocacy Coalition calls.
9. Participates in required trainings.
10. Conduct meetings & trainings on programs such as DPP Lifestyle Coaching & Healthy Heart Ambassador in communities for promotion, education and advocacy for reducing morbid conditions such as Diabetes, Hypertension, obesity and smoking in high risk populations or complications from COVID-19.
11. Attends meetings, submits reports and work plans, activity reports, reports on accomplishments, lessons learned & successes in a timely manner.
12. Be able to multi-task & be flexible with accomplishing the PH Mission while demonstrating empathy to patients with fears & hesitancy of healthcare systems.
13. Communicates well with others, exhibits a professional & positive attitude, shows up for work on time & ready to get the job done.

Minimum Qualifications

SST 2:
Bachelor's degree in a social services related field from an accredited college or university OR High school diploma or GED AND Two years of experience in a social services related position OR One year of experience at the lower level Social Svcs Tech 1 (SST010) or position equivalent.

SST 3:
Bachelor's degree in a social services related field from an accredited college or university AND One year of experience in a social services related field OR High school diploma or GED AND Three years of experience in a social services related position OR One year of experience at the lower level Social Svcs Tech 2 (SST011) or position equivalent.

Preferred Qualifications:

  • Public Health education
  • Ability to read, write and speak English & Spanish
  • Experience in working within the healthcare or community organizations
  • Experience in delivering trainings on PH elated topics
  • Requires flexibility to travel up to 90% of the time to community sites
