Community Health Specialist- Arnett Hospital (Part Time/Temporary)

Overview

This position will be located at Arnett Hospital in Lafayette, IN and is part time and temporary. This position provides community based outreach work to populations at risk for HIV and Hepatitis C. Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.

• High School Diploma or equivalent required.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
