Job Description

The Division of Fee for Service Management (DFSM) is looking for a highly motivated individual to join our team. The Clinical Care Specialist is responsible for supporting the Nurse Care Coordinator in the High Needs High Cost (HNHC) initiative. The primary responsibility is to ensure each HNHC member’s information is being shared with the correct regional stakeholder at the right time. The Clinical Care Specialist is responsible for sharing weekly eligibility updates with the HIE and the Data Analysis and Research area of the Division. The position also works with care coordination partners, including Tribal Regional Behavioral Health Authorities (TRBHAs) and Regional Behavioral Health Authorities (RBHAs). The Major duties and responsibilities include but are not limited to:

  • Provides support to the CMSU Care Coordination functions, including documentation for high needs/high-cost clinical staffing's, member eligibility, member rolls, and maintaining care coordination eligibility.

  • Responds to partner inquiries related to care coordination process, and other miscellaneous issues.

  • Develop and implement CMSU infrastructure related to care coordination, including desk levels, process, and data flows, serve as lead for care coordination platforms, including the HIE and SharePoint.

  • Cross train CMSU staff in care coordination functions.

  • Provides support to the DFSM Continuation of Operations Plan (COOP) lead.

  • Provides technical assistance to the TRBHAs/RBHAs on accessing SharePoint.

  • The lead in helping the Technical Coordination Unit (TCU) on member transfers.
Knowledge, Skills & Abilities (KSAs): Knowledge:
  • Knowledge of mechanisms necessary for coordination and delivery services.

  • Knowledge of the Healthcare Delivery System.

Skills:
  • Strong organizational skills for setting priorities, workload and record keeping.

  • Skills with report developing/writing.

Abilities:
  • Ability to assess variables which impact health and functioning.

  • Ability to read, interpret and apply complex rules and regulations.
Qualifications: Minimum:
  • Bachelor in behavioral health, social science, or three years experience in behavioral health or healthcare.

Preferred:
  • Experience in working with Tribal facilities and the various health care delivery systems in which Tribal members may receive services.
  • Previous experience working in care coordination.
Pre-Employment Requirements:
  • Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees.
  • Successfully pass fingerprint background check, prior employment verifications and reference checks; employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions.
  • Travel may be required for State business. Employees who drive on state business must complete any required driver training (see Arizona Administrative Code R2-10-207.12.) AND have an acceptable driving record for the last 39 months including no DUI, suspension or revocations and less than 8 points on your license. If an Out of State Driver License was held within the last 39 months, a copy of your MVR (Motor Vehicle Record) is required prior to driving for State Business. Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed.
