HOURS
Casual. Rotating shifs. Varied schedule. No guaranteed hours per pay period. FTE 0.0.

SUMMARY
Provides primary nursing care through patient triage, prescription refills, management of the INR patient care, follow-up on depression and telemedicine. Demonstrates environmental concerns of infection control practice, safety, fire practice, disaster practices, cardiac arrest, and CQI.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Telephone Triage
Uses effective communication skills to obtain pertinent patient information to identify the appropriate patient protocol, need for a consultation, or other action.

Prescription Refill Program
Follows a Provider approved Refill Protocol to refill patient medications.

INR Anticoagulation Clinic:
Follows the Provider approved Protocol to dose patients that are on blood thinners.

Depression Protocol:
Meets with patients that have a new diagnosis of Depression and have a desire for one on one conversation with an RN. Follows a Protocol approved by the Providers of prescribed treatment and therapies within the scope of the RN. Meets with the patient for a total of 6 sessions.

Support Quality Measures
RN will call patients with Depression that are due for a follow up appointment. They will use a report that lists a range of dates that the patient needs to be seen or should have been seen. They will administer a PHQ-9 test over the phone to the patient and also gather information from the patient as to their status. Based on the RN assessment the patient will be scheduled for an appointment with their PCP if appropriate.
RN will see patients that have been called by Nurses and/or Receptionist and told that they need to have their blood pressure check. RN is able to answer any questions and provide patient teaching about blood pressure and smoking cessation.
RN will also review the lab schedule for the next day for the clinic. This will ensure that the patient is having the labs drawn that they need for their medications and also for the quality initiatives.

Insulin Quik-Starts
RN will instruct a Diabetic patient how to give themselves insulin injections following a process written by the FirstLight Health System CDE.

Impact Concussion Testing
Provides support to the Providers and/or patients participating in the ICT Program.

Tele-Med
Assists the patient and Provider in setting up and completing these appointments.

Walk In Triage & Emergencies
RN will be available at all clinic sites during business hours to provide assessment of walk in injuries, illness and emergencies.

Demonstrates customer service skill to positively impact the experience of care for our patients, external customer and internal service partners.

JOB RELATIONSHIPS
This position reports directly to the Clinical Services Manager for overall supervision, and to the Clinical Service Manager, Nurse Practitioner, or Physician for medical supervision and consultation. The RN works with patients, families, physicians and other employees in addition to pre-hospital personnel.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Graduate of an accredited school of nursing. Current MinnesotaRN License. Additional requirements for this position are three to five years nursing experience in medical surgery or an appropriate specialty area. Experience in an ambulatory care setting and one+ year of triage preferred. Computer/keyboard familiarity helpful. Maintain BLS certification. Knowledge and ability to employ the principles, tools, and methods of TQI. Excellent customer service, verbal and written communication skills. Ability to handle fluctuating volume of calls and problem solve. Ability to assess and respond to various patient needs quickly and efficiently. Demonstrate ability to organize and exercise judgment. Ability to learn keyboard / computer system.
