Exciting news - our Team Leads now work (3) 12 hour shifts! Full-time shift is 6:00 am - 6:30 pm 3 days a week. The Team Lead-CNA is responsible for overseeing the provision of healthcare services for Residents with intellectual disabilities in the campus homes, and within the Adult Training Facility, when necessary. Team Leads will oversee the improvement of daily living and report any issues to the charge nurse or the ADON; for providing quality, continuous personal care to our Residents; and for mentoring the DSP staff who provide direct care services to our individuals. You will be part of a team dedicated to making a difference in the lives of others. This is a FT, non-exempt position at our Main campus. Hourly rate is $17.00 hr. and there are added differentials for overnight, up to $17.50 hr. plus a FT enhanced wage option. AND - There is an additional $1000 sign-on bonus!
Essential Responsibilities

  • Observe, assist, and mentor while working hands on with Direct Support Professionals (DSPs) in order to provide support for maximum independence of Residents.
  • Instruct DSPs in assisting the Residents with activities of daily living to include feeding, dressing, toileting, bathing, transporting, mobility, turning and positioning, oral hygiene, shaving, nail care, as well as performing other healthcare tasks.
  • Performs tasks to assist DSPs with direct Resident care including feeding, dressing, toileting, bathing, transporting, mobility, turning and positioning, oral hygiene, shaving, nail care, and well as other healthcare tasks.
  • Provide, obtain, and perform direct patient care to include skin evaluations, blood pressure checks, collection of specimens, intakes and outputs, ambulation, meal consumptions, height & weight checks, glucometer readings, bladder scans , G-tube monitoring and maintenance, and equipment calibration. Assist RN/LPN with positioning of Residents during medication administration.
  • When an LPN/RN is not present in the home, the Team Lead will be responsible for disconnecting and reconnecting a G-Tube when individuals are being attended.
  • Obtain and record vital signs; obtain urine and stool sample when requested by the Nursing Department.
  • May perform venipuncture for lab draws, if experienced and certified in phlebotomy.
  • Assist with transport preparation; accompany Residents to medical appointments.
  • Monitor and assists DSPs in completing activities with Residents as outlined in an Individual Treatment Plan under the guidance of a House Manager and/or QIDP staff.
  • Ensure DSPs are following the proper meal and bathing procedures.
  • Educate and ensure DSPs take and document water temperatures before each bath or shower to prevent incidents of burns.
  • Participate in meal preparation and feeding to ensure adherence to proper food and liquid consistencies; and ensure proper positioning before, during, and after meals for all Residents, especially for Residents with risk of aspiration.
  • Observe Residents for physical and psychosocial needs, attitudes, reactions, etc., and report unusual findings to the Charge Nurse.
  • Ensure DSPs observe behaviors and changes in physical condition of Residents; to utilize prescribed strategies to assist the Residents; and to document accordingly.
  • Participate in interdisciplinary team meetings in order to provide feedback regarding direct observations of individual residents to professional staff.
  • Participate in DSP’s work performance consistent with operational or organizational requirements.
  • Ensure DSPs are accurately documenting and maintaining logs/records pertaining to Residents’ behavioral issues.
  • Communicate concerns to appropriate supervisors as needed.
  • Assist DSPs to make certain that living areas are clean and safe.
  • Maintain professionalism and confidentiality when communicating with co-workers and families of Residents.
  • Perform other duties as required.

Required Experience:

  • Must possess High School Diploma or G.E.D.
  • Must have an active PA Certified Nursing Assistant (CNA) license, having successfully completed a State approved Nurse’s Aide Training Program and Competency Evaluation.
  • Applicants must be 18 years of age, maintain a valid driver's license, and have access to a vehicle
  • Must successfully complete required clearances, physical, and employment verification, and maintain a valid Driver’s License.
  • Must have at least 6 months clinical experience as a Certified Nursing Assistant (CNA).
  • Must have ability to comprehend reports and regulations, speak and provide written documents in an understandable manner.
  • Must demonstrate flexibility and initiative in job responsibilities.
  • Experience with ID/D individuals preferred.
  • Leadership and supervisory experience preferred

Physical Requirements
Requires ability to lift 50 pounds independently, see, hear, speak, stand, bend, walk, push, pull, climb, and reach. This position requires sitting, standing, walking and driving
