Job Description
Overview:Program Description:The Wintonia is a 92-unit SRO community serving very low-income adults who are homeless at time of move-in. Applicants are generally alcohol-dependent and may have co-occurring disorders and histories of trauma. Referrals to the Wintonia are primarily from the Dutch Shisler Sobering Center. The Wintonia provides permanent supportive housing within a Housing First and Harm Reduction model.
The Wintonia team works from a multi-disciplinary approach in which all staff coordinate services to promote housing stability in an environment that honors and respects the dignity of each resident.
Position Description:
The Wintonia Case provides case management and partners with other service providers to achieve housing stability in an environment that promotes individual dignity and respect. This position works one weekend day. The scheduled work week is either Tuesday through Saturday or Sunday through Thursday.Responsibilities:MAJOR DUTIES AND RESPONSIBILITIES
Case Management:

  • Demonstrate a thorough understanding and operate from a philosophy of harm-reduction and trauma–informed care, including a primary utilization of motivational interviewing techniques and a strength based person centered case management approach.
  • Responsible for a designated caseload of approximately 40 individuals and assist those residents with tasks needed to improve quality of life and/or daily living skills.
  • Responsible for maintaining excellent communication, advocacy and care collaboration efforts to promote resident well-being, personal growth and housing stability thru informal and formal meetings and goal setting.
  • Promote and provide referrals to all appropriate community resources and assist residents with completing appropriate entitlement paperwork; including benefits and housing applications.
  • Meet all agency requirements for documenting in resident files and online HMIS reporting, and complete/update intakes, assessments, and Housing Service Plans with each resident
  • Collaborate with clients on the Alcohol Management Program to create an alcohol distribution plan that best fits the client’s needs.
  • Assist residents with tracking of appointments and transportation; as well as accompanying residents when appropriate to appointments.
  • Initiate engagement with clients through intensive outreach, activities, groups, or other events that promote a healthy community as time allows.
  • Tolerate unusual or unpleasant behavior(s) and provide de-escalation and crisis intervention with vulnerable and disabled individual- who may have current issues with, or long histories of trauma, substance abuse, mental health and/or chronic medical issues and/or be in an ongoing state of distress.
  • Make home-visits to resident’s apartments to ensure resident welfare and to confirm the apartment is in a safe and homely state and to confirm unit is in compliance with all city and federal housing quality standards.


Supportive Services/Resident Relations:

  • Participate in Wintonia team meetings, case management client staffings and care conferences. As appropriate, ensure all appropriate eviction prevention tactics have been explored thru direct coordination and close cooperation with supervisors and property management staff.
  • Work closely with peers, direct supervisors and administrative staff to contribute to a community that celebrates and demands safety and dignity for all different ethnic identities, races, sexual orientations, gender identities served.
  • Assist with the scheduling and coordination of services of visiting professionals, including public health and visiting nurses, mental health counselors, chemical dependency counselors and chore worker.
  • Work with the Coming Home Program Case Manager on completing Seattle Housing Authority and Catholic Housing Services applications to fill vacancies.
Other:
  • Maintain regular, scheduled office hours.
  • Attend trainings required of employees and supervisory meetings as scheduled.
  • Represent Wintonia and CCS in service provider meetings or events, advocacy groups, coalitions, etc.
  • Cultivate and support a positive team-oriented work environment by responding to all emails, assisting team members and communicating clearly any information that may be beneficial for the program team to know.

JOB CONDITIONS
This position requires the employee to work in an environment where there may be exposure to blood, bodily fluids, and other potentially infectious material. Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance. MINIMUM QUALIFICATIONS
: Qualifications which applicants must possess to be considered for this position.
  • Minimum one year of work experience working with homeless or formerly homeless, drug addicted and/or mentally ill adults.
  • Demonstrated skills in problem solving, issue assessment and crisis intervention with ability to maintain client records.
  • Ability to communicate effectively, verbally and in writing while maintaining a professional and positive demeanor.
  • Experience working in residential facility or an understanding of residential settings.
  • Basic computer skills with ability to maintain client records, produce community resource information flyers and informational materials.
  • Ability to work independently as well as in a team environment and working as part of a multidisciplinary program.
  • Demonstrate experience and knowledge of mental health, domestic abuse, drug/alcohol addictions, crisis intervention and other issues related to homelessness.
  • Ability to support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services.
  • Criminal history background checks are required prior to employment.
  • Support and contribute to a creative, collaborative and respectful environment that promotes teamwork.
  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
PREFERRED QUALIFICATIONS
  • BA degree in Social Service or related field.
  • Experience in crisis intervention and ability to maintain a professional demeanor.
Knowledge of local social services relevant to low income, homeless, drug addicted and/or mentally ill adults.
