As a Supervisor for our Case Management team you will mentor and support case managers; while being responsible for the coordination of care during the discharge process. You will assist with scheduling, training and development of staff while managing your own case load. You will perform assessments and assist other care coordinators with their assessments as needed. The Case Manager Supervisor will build new and maintain current relationships with organizations and partners outside the system to meet patient needs. This position is 80% Case Management and 20% mentoring, scheduling and other team support responsibilities as needed. As the patient advocate, you will be responsible for identifying the best solutions for the patient’s care upon discharge.

This is a Monday – Friday position.

There is a 6 to 8 week weekend rotation required.

Responsibilities:

  • Supervises daily Care Coordination operations as assigned by Care Progression Manager.
  • Partners in responsibility for all resources and functions related to Care Coordination and regulatory compliance adherence.
  • Works in collaboration with Care Progression Manager for training and development of staff.
  • Participates in hiring decisions, discipline and evaluations.
  • Remains key member of the Care Progression team by handling case load as warranted, while assisting staff.
  • Partners with team to achieve quality outcomes and progression with the departmental strategic plan.
  • Responsible for assuring the receipt of high quality, cost efficient outcomes for department.
  • Collaborates and communicates with entire multidisciplinary team to ensure appropriate progression through the continuum of care.

We are looking for a Registered Nurse with Case Management or Care Coordination experience who is passionate about helping others learn and grow their skills. We are looking for a strong patient advocate who is solution focused, can perform issue and conflict resolution calmly while keeping the patient first, and possesses strong multi-tasking and time management skills.
