Job Description

Avivo Recovery Services Division provides a continuum of integrated services, including access, clinical treatment, care coordination and recovery support. We provide services that are innovative, person centered, strength based, and outcome driven. Under the direction of the Family Program Director, the Case Manager –SAMHSA Re-Entry, is responsible for access, reentry, and case management services to individuals, primarily to ex-offenders, who are participating in chemical health treatment and recovery maintenance/continuing care beginning with in reach while incarcerated and transitioning to treatment and recovery services in the community.

ESSENTIAL RESPONSIBILITIES:

  • Demonstrate commitment to SAMSHA’s definition of recovery and recovery principles: “A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.” www.samhsa.gov
  • This Case Manager role will be required to provide service to clients in correctional facilities, in the community, and in participant homes.
  • Meet with clients while incarcerated and develop, monitor and update reentry plans, as needed.
  • Provide on going assessment and referral utilizing prior knowledge and understanding of mental health, chemical health, housing, community resources, and case management, etc.)
  • Employ a holistic approach including promoting health and wellness activities with participants.
  • Actively participate in the division’s Trauma Informed Care Initiative and motivational interviewing. Support and implement evidence based practices related to specific program area.
  • Participates as a partner on the team; contribute to discussions, actions and projects. Maintains a favorable working relationship with all other employees to foster and promote a cooperative and harmonious working climate, conducive to maximum employee morale, productivity, efficiency, and effectiveness.
  • Demonstrates a positive and collaborative team approach by endorsing the same purposes and focuses as the team while encouraging trust and cooperation. Works within the team to assure that all benchmarks and goals are achieved and performs as a productive team member who supports the mission.

  • Meets performance criteria as outlined in annual performance evaluation and individualized performance standards.
  • Bachelors Degree or equivalent experience.
  • Previous work experience in a behavioral health setting.
  • Demonstrated understanding of the recovery philosophy and recovery oriented behavioral health services, or willingness to learn recovery principles.
  • Working knowledge of serving individuals with chemical and mental health or dual diagnoses.
  • Prior experience in a culturally diverse environment, working with individuals from varied backgrounds.
  • Demonstrated commitment to developing and delivering culturally responsive services.
  • Excellent participant-oriented, customer service attitude and communication skills.
  • Able to assess and solve problems, make decisions, work independently as needed.
  • Ability to work effectively and cooperatively with staff members at all levels of the organization and accept from supervisor and peers.
  • Willingness to be flexible and adaptable to changing service needs.
  • Able to maintain positive presence in a fast paced, demanding, multi service environment.
  • Ability to work independently with participants in home or community settings and capable of independent travel, have access to an automobile, and have a clean driving record.
  • Minimum one year freedom from alcohol and chemical abuse addiction.
  • Available to work outside of normal business hours or on call in order to meet the needs of the participants.

WAGES: $19.45 - $24.31/hour, DOQ
